牙周炎基础治疗用于慢性牙周炎伴糖尿病疗效分析

Treatment of chronic periodontitis with diabetes mellitus by basic therapy of periodontitis

目的分析牙周炎基础治疗用于慢性牙周炎伴糖尿病疗效分析。方法按本研究各项标准纳入60例慢性牙周炎伴糖尿病的老年患者,用双盲法分为2组,对照组行常规药物治疗和口腔清洁,观察组在此基础上行牙周炎基础治疗。以治疗前、治疗3个月、治疗6个月为观察时间点,对比2组牙周探诊深度(PD)、附着丧失(AL)、菌斑指数(PI)、出血指数(BOP)、糖化血红蛋白(Hb A1C)、空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)水平。结果治疗前2组患者上述指标均无显著差异(P>0.05);治疗3个月时,2组PD、AL、Hb A1c、FPG、TG、TC均无显著差异(P>0.05),但观察组PI、BOP指数较对照组低(P<0.05);治疗6个月时,2组AL、Hb A1C、TC均无显著差异(P>0.05),但观察组PD、PI、BOP、FPG、TG均低于对照组,P均<0.05。结论牙周炎基础治疗用于慢性牙周炎伴糖尿病老年患者有以下特点:(1)从第3个月起开始显著改善PI、BOP指数,但总体效果不明显,在6个月时达到最佳效果;(2)能显著提高患者牙周健康程度;(3)虽然对患者血糖、血脂有积极影响,但整体效果并不明显。

Objective To analyze the therapeutic effect of periodontal basic therapy on chronic periodontitis with diabetes mellitus. Methods 60 elderly patients with chronic periodontitis and diabetes mellitus were included in this study. These patients were divided into two groups by double - btind method. The control group were treated by self - intervention. The observation group were treated with periodontitis. PD, loss of attachment （ AL ）, plaque index （ PI ）, bleeding index （ BOP ）, glycosylated hemoglobin （ Hb ）, HbAlc, fasting blood glucose （FPG）, triglyceride （TG）, total cholesterol （ TC ） levels were measured before treatment, three months and six months as observation time points. Results There was no significant difference between the two groups before treatment （ P 〉 0.05 ）. There was no significant difference in PD, AL, HbA1c, FPG, TG and TC between the two groups （ P 〉 0.05 ）, BOP index was lower than that in Control group （P 〈 0.05 ） ;after treatment for 6 months, the two groups AL, HbA1C, TC were no significant difference （P 〉 0.05 ）, but the observation group PD, PI, BOP, FPG, TG were lower in the control group （P 〈 0.05 ）. Conclusion The basic treatment of periodontitis for chronic periodontitis with diabetes in elderly patients have the following characteristics ： firstly, from the first 3 months began to significantly improve the PI, BOP index, but the overall effect is not obvious, at 6 months to achieve the best Effect; secondly,it can significantly improve the degree of periodontal health; thirdly, although the blood glucose, blood lipids have a positive impact, but the overall effect is not obvious.
