脾氨肽对肺炎支原体感染致哮喘患者淋巴细胞亚群的影响研究 被引量:16

摘要 目的分析脾氨肽冻干粉口服治疗肺炎支原体(MP)感染所致哮喘发作患者的临床疗效,为MP感染哮喘发作患者临床治疗提供参考。方法选择2015年3月-2016年3月医院治疗的MP感染致哮喘发作患者108例,采用随机数字表法将患者分为对照组和试验组,每组54例,对照组患者采用常规对症治疗及抗菌药物治疗,试验组患者在对照组治疗方案基础上,加用脾氨肽冻干粉口服治疗,两组患者均连续治疗2周,比较两组患者入组、治疗后免疫球蛋白IgG、IgA、IgM、T淋巴细胞亚群CD3+、CD_4^+、CD_8^+ 、CD_4^+/CD_8^+ 变化情况,并评价两组患者的临床症状评分。结果治疗2周后,两组患者IgG、IgA较治疗前均升高,IgM较治疗前降低(P<0.05),但试验组患者治疗后IgG、IgA高于对照组患者,IgM低于对照组患者(P<0.05);两组患者治疗后CD3+、CD_4^+、CD_4^+/CD_8^+ 较治疗前升高(P<0.05),CD_8^+ 较治疗前明显降低(P<0.05),但试验组患者治疗后CD3+、CD_4^+、CD_4^+/CD_8^+ 高于对照组患者(P<0.05),CD_8^+ 低于对照组患者(P<0.05);两组患者治疗后哮喘临床症状评分均较治疗前有明显降低,但治疗后试验组哮喘临床症状评分明显低于对照组(P<0.05)。结论脾氨肽冻干粉口服辅助治疗可有效降低MP感染致哮喘发作患者哮喘临床症状评分,改善患者T淋巴细胞亚群构成,提高患者免疫功能,对MP感染致哮喘患者的治疗具有较高的临床价值。 OBJECTIVE To investigate the clinical efficacy of spleen aminopeptide oral lyophilized powder in treatment of patients with asthma induced by Mycoplasma pneumoniae(MP)infections so as to provide guidance for clinical treatment of the patients with asthma induced by MP infections.METHODS A total of 108 patients with MP infection-induced asthma who were treated in the hospital from Mar 2015 to Mar 2016 were enrolled in the study and were randomly divided into the control group and the observation group,with 54 cases in each group.The control group was treated with conventional symptomatic therapy and antibiotics,while the observation group was given additional spleen aminopeptide oral lyophilized powder on basis of the treatment of the control group,both groups of patients were treated for 2 consecutive weeks.The levels of immunoglobulins IgG,IgA,IgM,T lymphocytes subsets CD3+,CD4^+,CD8^+ and CD4^+/CD8^+ were observed and compared between the two groups of patients before and after the treatment,and the clinical symptoms scores of the two groups of patients were evaluated.RESULTS The levels of IgG and IgA of the two groups of patients were elevated after the treatment for 2 weeks,and the IgM level was reduced(P〈0.05);the levels of IgG and IgA of the observation group were higher than those of the control group after the treatment,and the IgM level of the observation group was lower than that of the control group(P〈0.05).The levels of CD3+,CD4^+and CD4^+/CD8^+ of the two groups of patients were higher after the treatment than before the treatment(P〈0.05),the CD8^+ level was lower after the treatment than before the treatment(P〈0.05);the levels of CD3+,CD4^+and CD4^+/CD8^+ of the observation group were higher than those of the control group after the treatment,and the CD8^+ level of the observation group was lower than that of the control group(P〈0.05).The clinical symptoms scores of the two groups of patients were remarkably lower after the treatment than before the treatment,however,the clinical symptoms scores of the observation group were significantly lower than those of the control group after the treatment(P〈0.05).CONCLUSIONThe spleen aminopeptide oral lyophilized powder can effectively reduce the clinical symptom score of asthma of the patients with MP infection-induced asthma,improve the constituent ratios of T lymphocytes subsets and boost the immune function,and it has high clinical value in treatment of the patients with asthma induced by the MP infection.
