目的探讨3.0T MRI在剖宫产瘢痕妊娠(CSP)诊断中的价值。方法对临床疑诊CSP患者30例术前行MRI检查,以术后病理结果为金标准,分析MRI对早期CSP诊断的敏感性、特异性、准确性。结果 30例疑似CSP患者中,MRI正确诊断28例,诊断敏感度为96.4%,特异度为50%,阳性预测值96.4%,阴性预测值为50%,诊断符合率93.3%。结论 MRI对早期CSP的正确诊断率较高,可明确妊娠组织的部位、大小、峡部厚度以及与周边组织结构的关系,增强扫描可明确病变血供情况,对切口妊娠的诊断、指导和制定手术方案具有一定的价值。

Objective To study the value of the 3.0 T MRI findings in the early diagnosis of cesarean scar pregnancy （CSP）. Methods The 30 patients with clinical suspected CSP underwent MRI examination. Taking the postoperative pathological exami- nation results as the gold standard of MRI results with the control, we analyzed the sensitivity specificity and accuracy of early di- agnosis of CSP by MRI. Results 28 cases were correctly diagnosed by MRI. The diagnostic sensitivity was 96.4% , the specific- ity was 50% , the positive positive predictive was 96.4% , the negative predictive was 50%. The diagnostic accordance rate was 93.3%. Conclusion Early CSP correct diagnosis rate by MIR is higher, which can clearly show pregnancy tissue site, size, thickness of the isthmus and relationship with surrounding structures. MRI is valuable for diagnosing CSP.
