比较两种填充敷料在负压伤口治疗中的应用效果

Effect comparison of two filling dressings in negative pressure wound therapy

目的比较两种填充敷料在负压伤口治疗中的应用效果。方法将入组的84例患者随机分为试验组和对照组,每组各42例,分别采用曲缩棉和聚氨酯泡沫敷料填充后应用负压进行伤口治疗。比较两组患者伤口愈合情况、更换敷料时伤口疼痛程度和二次创伤评分、更换敷料耗时数、敷料易用性、治疗时间和治疗费用。结果试验组在伤口疼痛程度评分(4.16±0.70)分、更换敷料时二次创伤评分(1.14±0.32)分、更换敷料耗时数(12.84±1.31)min、敷料易用性(8.96±1.24)分、治疗费用(623.33±25.16)元方面均优于对照组(6.47±0.56)分、(2.51±0.19)分、(17.83±1.45)min、(6.32±1.12)分、(1 196.67±20.81)元,差异均有统计学意义(P<0.05),而试验组和对照组在愈合情况、治疗时间[(27.65±4.38)d、(26.24±5.13)d]方面相近,差异无统计学意义(P>0.05)。结论曲缩棉和聚氨酯泡沫敷料均可作为负压伤口治疗的有效填充敷料,但曲缩棉给患者带来的二次创伤和疼痛感更小、使用更便捷、成本更低。

Objective To compare the effects of two filling dressings in negative pressure wound therapy.Methods84 patients were randomly divided into the experimental group and the control group,with 42 cases in each group.Gauze sponge and polyurethane foam dressing were used respectively to fill after the application of negative pressure wound therapy.The wound healing,degree of wound pain at dressing change and secondary trauma scores,time for dressing change,dressing ease of use,treatment time and cost of treatment were compared between the two groups.Re s ults The experimental group were better than the control group in wound pain degree scores at dressing change[（4.16 ±0.70） VS（6.47 ±0.56）],secondary trauma scores at dressing change [（1.14 ±0.32） VS（2.51 ±0.19）],time for dressing change [（12.84 ±1.31） VS（17.83 ±1.45）],dressing ease of use [（8.96 ±1.24） VS（6.32 ±1.12）]and cost of treatment [（623.33 ±25.16） VS（1196.67 ±20.81）] and the differences were statistically significant（ P〈0.05）.The experimental group and control group were similar in wound healing,treatment time [（27.65 ±4.38） VS（26.24 ±5.13）] and the differences were not statistically significant（P〈0.05）.Conclus ion Both gauze sponge and polyurethane foam dressing can be used as an effective filling dressing for negative pressure wound therapy but gauze sponge brought less pain to patients and less secondary trauma.It was more convenient to use and cost-efficient.
