醋酸亮丙瑞林预处理联合腹腔镜下子宫肌瘤剔除术治疗巨大子宫肌瘤的效果评价

Evaluation on the efficacy of Leuprorelin Acetate pretreatment combined with laparoscopic myomectomy in the treatment of giant uterine fibroids

目的探讨醋酸亮丙瑞林预处理联合腹腔镜下子宫肌瘤剔除术治疗巨大子宫肌瘤的临床疗效。方法选取2014年5月~2016年5月我院收治的60例巨大子宫肌瘤患者作为研究对象,依照入院顺序分为两组,每组各30例。常规组患者直接采取腹腔镜下子宫肌瘤剔除术,实验组患者给予醋酸亮丙瑞林预处理后进行手术治疗。比较实验组患者治疗前后的子宫肌瘤体积,比较两组患者的手术情况、术后恢复情况、复发率及妊娠率。结果实验组患者治疗后的子宫肌瘤体积明显小于治疗前,差异有统计学意义(P<0.05)。实验组患者的术中出血量明显少于常规组,下床活动时间、盆腔引流时间、住院时间均明显短于常规组,差异有统计学意义(P<0.05)。两组患者的复发率、妊娠率比较,差异无统计学意义(P>0.05)。结论对巨大子宫肌瘤患者采取醋酸亮丙瑞林联合腹腔镜下子宫肌瘤剔除术进行治疗,效果显著,便于减少术中出血量,缩短住院时间,利于身体的早日康复。

Objective To investigate the clinical efficacy of Leuprorelin Acetate pretreatment combined with laparoscopic myomectomy in the treatment of giant uterine fibroids.Methods 50 patients with giant uterine fibroids who were treated in our hospital from May 2014 to May 2016 were selected as subjects.According to the order of admission,the patients were divided two groups,30 cases in each group.The conventional group was directly given laparoscopic myomectomy,and the experimental group was given Leuprolide Acetate pretreatment before the surgical treatment.The volume of uterine fibroids were compared in the experimental group before and after the treatment,and the surgical conditions,postoperative recovery,the recurrence rate and the pregnancy rate of the two groups were compared.Results The volume of uterine fibroids in the experimental group after treatment was significantly lower than that before treatment, and the difference was statistically significant （P〈0.05）.The intraoperative blood loss in the experimental group was less than that in the conventional group,time of off-bed activity,pelvic drainage time and length of hospital stay in the ex- perimental group were all significantly shorter than those in the conventional group,and the differences were statistically significant （P〈0.05）.There was no significant difference between the two groups in recurrence rate and pregnancy rate （P〉0.05）.Conclusion The effect of Leuprolide Acetate combined laparoscopic myomectomy is significant in the treatment of patients with giant uterine fibroids,which is easy to reduce intraoperative blood loss,and shorten the length of hospital stay,and is conducive to early recovery.
