摘要 目的:探讨MSCT胸部血管成像不同扫描方案的应用价值。方法:临床怀疑胸部血管病变需做CTA检查的患者共90例,分成A、B、C三组,每组各30例。A组为肺动脉阈值触发扫描方案。B组为改良肺动脉阈值触发扫描。C组为主动脉阈值触发扫描方案。结果:肺动脉CT值A组和C组、B组和C组差异有统计学意义(P<0.01),A组与B组差异无统计学意义。降主动脉的CT值:A组与B组、A组与C组、B组与C组差异有统计学意义(P<0.01)。结论:A组适用于:咯血,或者无咯血,但肺部平扫大片实变段以上蜂窝肺的患者;B组适用于:无咯血,肺部平扫两肺无不张实变、无蜂窝肺的患者,用于排除有无肺动脉栓塞;C组适用于咯血、支气管动脉扩张的患者。根据临床提供的病史和检查目的选择正确的扫描方案,能更好地显示病变和提高诊断的准确性。 Objective:To evaluate different scan protocol in the thoracic MSCTA. Methods:90 patients suspected clinically as vascular lesions accepting thoracic CTA scanning were divided equally into A, B, C groups. Group A:the pulmonary artery threshold trigger scanning. Group B:the improvement of pulmonary artery threshold trigger scanning. Group C:the standard of aortic threshold trigger scanning solutions. Results:The difference of pulmonary artery CT values in group A and group C, group B and group C was statistically significant, respectively ( P〈0. 01 ) , but there is no statistically significant in Group A and group B. The descending aorta CT value in Group C was higher than that of group A and B, the differences was statistically significant. The difference of descending aorta CT values in group A and group B, group A and group C, group B and group C, was statistically significant respectively (P〈0.001). Conclusion:Group A applies to the patients with hemoptysis, no hemoptysis but the lung is scan large pieces of honeycombing and the pulmonary consolidation occur in segmental level or above. Group B applies to the patients with no hemoptysis, no pulmonary consolidation or atelectasis, no honeycomb lung, and exclude pulmonary embolism. Group C is suitable for the patients with hemoptysis or bronchial artery expansion. According to the clinical history and examination, the correct scan protocol can display the lesion better and improve diagnostic accuracy.
