实施医护联合查房在基层医院儿科病区效果研究

The effect of joint rounds of doctors and nurses in pediatric ward of primary hospital

目的:探讨基层医院儿科病区实施医护联合查房的效果评价。方法:从2015年11月至2016年10月实施医护联合查房作为观察组,并与2014年11月至2015年10月传统医生、护士分开查房作为对照组,对临床路径临床应用、患者平均住院日、住院费用情况、患者满意度及儿科护士专科综合能力进行评价比较。结果:实施医护联合查房后,临床路径临床应用率从27.11%上升到43%,病人平均住院日从(5.6±3.5)天下降至(4.9±2.1)天,住院费用从(2274±780)元下降至(2135±325)元,患者满意度从86.02%上升至92.83%,护士专科综合能力优秀与良好人员从44%上升至89%,以上差异均有统计学意义(P<0.05)。结论:医护共同查房能提高临床路径的临床应用,减少患者住院天数及住院费用,提高病人满意度及儿科护士专科综合能力水平,对整个儿科团队建设与发展有一定的促进作用,值得临床推广应用。

Objective：To evaluate the effectiveness of the implementation of medical care and ward rounds in pediatric ward of primary hospital.Methods：From November, 2015 to October, 2016, we carried out the joint medical ward round as the observation group. From November, 2014 to October, 2015, the traditional doctors and nurses divided the ward round as the control group. The clinical application of the clinical pathway, Hospitalization, hospitalization costs, patient satisfaction and pediatric nurses comprehensive ability to evaluate the comparison. Results：The clinical application rate of clinical pathways increased from 27. 11% to 43%, and the average length of hospital stay decreased from （5.6±3.5） days to （4.9±2.1） days. The hospitalization cost was reduced from （2274±780） Yuan （92.53%）, the satisfaction rate of the patients increased from 86.02% to 92.83%, and the comprehensive and excellent nurses＇ professional competence increased from 44% to 89%. The difference was statistically significant （ P〈0.05） . Conclusion：The common medical ward can improve the clinical application of clinical pathways, reduce the length of hospitalization and hospitalization costs, improve patient satisfaction and the level of comprehensive competence of pediatric nurses, and promote the construction and development of the whole pediatric team, worthy of clinical application.
