治疗47例瘘管牙的临床分析

Clinical analysis of 47 cases with fistula teeth

目的探讨对于慢性牙槽脓肿中有瘘型患牙采用"双漏空感"方式疏通窦道内外口保存瘘管牙的可行性。方法对2013年10月-2017年7月近4年间在新兴街社区卫生服务中心口腔科治疗47例瘘管牙的结果做回顾性分析。结果经疏通窦道加完善根管充填,临床治愈后恢复正常咀嚼功能的有41例,其中观察超过2年治愈瘘管牙未复发的超过30例,超过1年复发4例经再治疗恢复咀嚼功能。治疗失败拔除2例。结论获得瘘管牙病灶根管根尖孔缩窄区及窦道在骨壁开口的漏空感是获得治愈成功的基础。打开循环通道加上正确的根管治疗,瘘管牙的保存率可达95%以上。

Objective To study the feasibility of ＂double leakage＂ way to save fistula teeth by dredging the internal and external mouth of the sious for on chronic alveolar abscess in teeth with fistula. Methods The results of 47 cases with fistula treated in Department of Stomatology of Xinxing Street Community Health Service Center from October 2013 to July 2017 were retrospectively analyzed. Results After the clear sinus and root canal filling,41 cases restored normal mastication function after clinical cure. More than 30 cases with healed fistula tooth without recurrence were observed for more than 2 years. 4 cases resumed mastication after retreatment which had a relapse after 1 year. 2 cases were removed with treatment failure. Conclusion It is the foundation for a successful cure to get the leak feeling of the root canal fistula dental focal apical constriction in the sinus area and bone wall opening. The preservation rate of the fistula tooth can reach more than 95% by opening the circular channel and adding proper root canal treatment.
