腹腔镜子宫全切术用于妇科良性肿瘤的效果及对机体免疫功能的影响

Effect of laparoscopic total hysterectomy on benign gynecologic tumor and its influence on immune function

目的探讨腹腔镜子宫全切术和开腹子宫全切术的效果以及对机体免疫功能的影响。方法选择本院2015年5月~2017年5月收治的妇科良性肿瘤病例150例,随机分为实验组与对照组,每组75例,实验组行腹腔镜全子宫切除术,对照组行开腹子宫切除术,比较两组术中与术后情况,并比较术前1d、术后3d的C3、IgG、IL-2和IL-4的水平差异。结果实验组术中出血量、住院天数以及术后排气时间均显著低于对照组(P<0.05);术后3d两组IgG、C3、IL-2均术前有明显降低(P<0.05),但实验组高于对照组(P<0.05),两组IL-4较术前具有显著升高(P<0.05),但实验组显著低于对照组(P<0.05);两组并发症差异无统计学意义(P>0.05)。结论腹腔镜子宫全切术治疗妇科良性肿瘤能够明显降低术中出血量,缩短术后排气时间,对患者机体免疫功能影响较小,适合推广使用。

Objective To investigate the effect of laparoscopie total hysterectomy and open hysterectomy and its influence on immune function of the patients. Methods 150 cases of benign gynecologic tumors who were treated in our hospital from May 2015 to May 2017 were selected and divided into experimental group and control group randomly,75 cases in each group.The experimental group were treated with laparoscopic total hysterectomy,and the control group were treated with open hysterectomy.The intraoperative and postoperative conditions of the two groups were compared.And the levels of C3,IgG,IL-2 and IL-4 were compared between 1 days before operation and 3 days after operation. Results The amount of bleeding,the number of days in hospital and the time of postoperative exhaust were significantly lower in the experimental group than that in the control group（P 〈 0.05）;3 days after operation,IgG,C3 and IL-2 of the two groups were significantly lower（P 〈 0.05）,which of the experimental group was higher than that of the control group（P 〈0.05）,the IL-4 of two groups was significantly higher than that before operation（P 〈 0.05）,which of the experimental group was lower than that of the control group（P 〈 0.05）;There was no significant difference between the two groups（P 〉 0.05）. Conclusion Laparoscopic total hysterectomy for gynecological benign tumors can significantly reduce the amount of intraoperative bleeding,shorten the postoperative exhaust time,and have little influence on the immune function of patients,so it is suitable for popularization and use.
