综合干预降低住院精神病患者约束率的效果研究

Effect of comprehensive intervention on reducing the restriction rate of hospitalized psychiatric patients

目的探讨综合干预策略在降低住院精神病患者约束率的应用效果。方法某精神病医院急性干预科600例住院精神病患者作为研究对象,随机分为实验组和对照组,每组300例,对照组采用精神科常规护理,实验组在常规护理基础上进行综合干预,比较两组患者约束率、平均约束次数、约束持续时间、满意度及意外事件发生例次。结果实验组约束使用率为45.3%、约束持续时间为(4.10±1.43)h、患者约束次数为(2.34±1.14)次,均低于对照组,差异均有统计学意义(P<0.05),实验组患者医患沟通(28.68±2.54)分、整体满意度(8.59±0.99)分得分高于对照组,差异有统计学意义(P<0.01),不良事件发生例次差异无统计学意义(P>0.05)。结论综合干预策略能降低住院精神病患者约束率,提高患者满意度,实现"最小化身体约束"的临床管理目标。

Objective To explore the effect of comprehensive intervention strategy on reducing the restriction rate of hospitalized psychiatric patients. Methods 600 hospitalized psychiatric patients in a psychiatric hospital were randomly divided into an experimental group and a control group, with 300 cases in each group. The patients in control group adopted general psychiatric nursing, and the patients in experimental group had a comprehensive intervention on the basis of conventional nursing. The rate of restraint, the number of constraints, the duration of constraint, the degree of satisfaction and the occurrence of accidents were compared between the two groups. Results In the experimental group, the restraint utilization rate was 45.3%, the duration of constraint was （4.10 ± 1.43） h, and the number of patients was （2.34 ± 1.14） times, which were all lower than those in the control group, and the differences were statistically significant（P 〈 0.05）. The scores of doctor-patient Communication was （28.68 ± 2.54） and overall satisfaction was （8.59±0.99） and which were higher in the experimental group than those in the control group. The differences were statistically significant （P 〈 0.01）. There was no significant difference in the occurrence of adverse events （P 〉 0.05）. Conclusion The comprehensive intervention strategy can reduce the rate of hospitalization and improve the patient＇s satisfaction, and achieve the goal of ＂minimizing body constraint＂.
