关节镜下射频消融与手术治疗膝关节创伤性滑膜炎临床效果对比

Comparison of clinical effect of arthroscopic radiofrequency ablation and arthroscopic surgery in the treatment of traumatic synovitis of knee joint

目的比较观察关节镜下射频消融系统治疗与关节镜下手术治疗膝关节创伤性滑膜炎的临床疗效,旨在为治疗方法的选择提供临床依据。方法以我院收治的76例膝关节创伤性滑膜炎患者为研究对象,随机分为观察组(38例,行关节镜下射频消融系统治疗)和对照组(38例,行关节镜下手术治疗),术后随访1个月,观察随访期间两组膝关节功能(Lysholms评分)、膝关节活动度改善情况及并发症发生情况。结果术后1个月,两组患者Lysholms评分、膝关节活动度均明显升高,且观察组Lysholms评分及膝关节活动度显著高于对照组;观察组膝关节功能优良率明显高于对照组(86.84%vs 71.05%),并发症发生率显著低于对照组(5.26%vs 15.79%),差异均有统计学意义(P<0.05)。结论相比于关节镜下手术治疗,关节镜下射频消融系统治疗能够有效改善患者膝关节功能和膝关节活动度,且安全性较高,有助于患者的术后康复。

Objective To compare clinical effect of arthroscopic radiofrequency ablation and arthroscopic surgery in the treatment of traumatic synovitis of knee joint,in order to provide clinical basis for the selection of therapeutic methods. Methods 76 cases with traumatic synovitis of knee joint in our hospital were selected as the research object and randomly divided into observation group（38 cases underwent arthroscopic radiofrequency ablation treatment） and control group （38 cases underwent arthroscopic surgery）;The patients were followed up for 1 month,and the knee function （Lysholms score）,knee mobility improvement and complications were observed in the two groups during the follow-up period. Results 1 month after surgery,Lysholms score and knee joint activity of two groups were significantly increased,and the observation group Lysholms score and knee joint activity was significantly higher than the control group.The knee joint function excellent rate of observation group was significantly higher than the control group （86.84% vs 71.05%）,the incidence of complications was significantly lower than the control group（5.26% vs 15.79%）,the differences were statistically significant（P 〈 0.05）. Conclusion Compared with arthroscopic surgery,arthroscopic radiofrequency ablation system can effectively improve the knee function and knee mobility,and the safety is high,which is conducive to the postoperative rehabilitation of patients.
