INTERTAN与PFNA内固定治疗股骨粗隆间骨折的疗效比较

Comparison of INTERTAN and PFNA internal fixation in the treatment of intertrochanteric fracture of femur

目的比较INTERTAN与PFNA内固定治疗股骨粗隆间骨折的疗效。方法选择2014年1月~2017年1月收治的64例股骨粗隆间骨折患者作为研究对象,根据治疗方法不同分为两组,其中34例应用INTERTAN内固定手术治疗,设立为观察组,其余30例应用PFNA内固定治疗,设立为对照组,对比分析两组患者的手术时间、出血量、术后并发症情况及骨折愈合时间、髋关节功能Harris评分。结果观察组的平均手术时间长于对照组,术中出血量多余对照组,两组比较差异有统计学意义(P<0.05);观察组的术后住院时间与对照组比较,差异不显著;观察组的术后并发症发生率17.65%,明显低于对照组,两组比较差异有统计学意义(P<0.05)。观察组应用INTERTAN治疗的骨折愈合时间明显快于对照组,与对照组比较差异有统计学意义(P<0.05)。观察组的Harris评分与对照组比较差异有统计学意义(P<0.05)。结论 INTERTAN与PFNA内固定治疗股骨粗隆间骨折均取得了较好的临床治疗效果,但INTERTAN内固定与PFNA内固定比较,更具有骨折愈合快、术后并发症少等优势,值得广泛推广和应用。

Objective To compare the efficacy of INTERTAN and PFNA in the treatment of intertrochanteric fracture of femur. Methods From January 2014 to January 2017,64 patients with intertrochanteric fracture were selected as the research object and were divided into two groups according to the different treatment methods.34 cases were treated with INTERTAN fixation and as the observation group,the remaining 30 cases were treated with internal fixation of PFNA and as the control group.The operation time,bleeding volume,postoperative complications,fracture healing time and Harris score of hip function were compared and analyzed between the two groups. Results The average operation time of the observation group was longer than that of the control group,the intraoperative blood loss of the observation group was more than that of the control group,there was statistically significant difference （P 〈 0.05）.The postoperative hospital stay in the observation group compared with the control group,the difference was not significant,the incidence of postoperative complications in the observation group was 17.65%,significantly lower than that of the control group,the difference between the two groups was statistically significant（P 〈 0.05）.The fracture healing time of the observation group treated with INTERTAN was significantly faster than that of the control group,the difference was statistically significant（P 〈0.05）.The Harris score of the observation group was not significantly different from that of the control group（P 〈 0.05）. Conclusion INTERTAN and PFNA in the treatment of femoral intertrochanteric fractures were achieved good clinical effect,but INTERTAN internal fixation compared with PFNA internal fixation has the advantages of fast fracture healing and less postoperative complications,and is worthy of extensive promotion and application.
