鼻内镜鼻窦开放术治疗慢性鼻窦炎临床分析

Clinical analysis of endoscopic open sinus surgery in the treatment of chronic frontal sinusitis

目的探讨鼻内镜鼻窦开放术治疗慢性鼻窦炎的临床效果。方法选择116例慢性鼻窦炎患者,随机分为对照组及研究组,每组58例,研究组采用鼻内镜下鼻窦开放治疗,对照组采用鼻窦冲洗治疗,治疗12周后比较两组患者临床有效率、鼻纤毛传输功能及鼻通气总阻力,随访6个月比较两组患者复发情况。结果研究组临床有效率为98.2%,对照组临床有效率为87.9%,研究组临床有效率高于对照组,差异有统计学意义(P<0.05),治疗前两组鼻纤毛传输速度及通气总阻力无统计学意义(P>0.05),治疗后研究组鼻纤毛传输速度快于对照组,差异有统计学意义(P<0.05),鼻通气总阻力小于对照组,差异有统计学意义(P<0.05),随访期间研究组复发率为3.4%,对照组复发率为22.4%,研究组复发率低于对照组,差异有统计学意义(P<0.05)。结论鼻内镜鼻窦开放术治疗慢性鼻窦炎能够提高临床疗效,降低复发率,具有较好的治疗效果。

Objective To investigate the clinical effect of endoscopic open sinus surgery in the treatment of chronic frontal sinusitis. Methods 116 patients with chronic sinusitis were selected and randomly divided into the control group and the study group,58 cases in each group.The study group was treated with endoscopic sinus surgery,and the control group was treated with nasal sinus irrigation.After 12 weeks of treatment,the clinical efficiency,mucociliary transport function and total nasal ventilation resistance were compared between the two groups.After 6 months of follow-up,the recurrence of the two groups was compared. Results The clinical effective rate of the study group was 98.2%,and that of the control group was 87.9%.The clinical effective rate of the study group was higher than that of the control group （P 〈 0.05）.There was no significant difference in nasal mucociliary transmission rate and total ventilation resistance between the two groups before treatment （P 〉 0.05）.After treatment,the nasal mucociliary transmission rate of the study group was faster than that of the control group （P 〈 0.05）,and the total resistance of nasal ventilation was less than that of the control group （P 〈 0.05）. During the follow-up period,the recurrence rate was 3.4% in the study group and 22.4% in the control group,and the recurrence rate in the study group was lower than that in the control group （P 〈 0.05）. Conclusion Endoscopic sinus surgery for chronic sinusitis can improve the clinical efficacy,reduce the recurrence rate,and has a good therapeutic effect.
