miRNA-499基因多态性与慢性丙型肝炎干扰素疗效的相关性研究

Association Study of miRNA-499 Gene Polymorphism with Interferon Efficacy in Chronic Hepatitis C

目的探讨miRNA-499基因多态性与慢性丙型肝炎干扰素疗效的相关性。方法选取我院141例丙型肝炎病毒(HCV)患者,所有患者皮下注射聚乙二醇干扰素α-2a注射液135~180μg,利巴韦林900~1200 mg/d,疗程为48~72周,疗程结束后miRNA-499基因双色荧光MGB探针检测rs3746444位点的多态性,根据miRNA-499基因多态性分为CC组104例和TT37例组,并判断基因多态性与干扰素疗效的关系。结果 141例HCV患者miRNA-499基因CC型检出104例,TT型检出37例,检出频率分别为73.6%、26.2%,未检出CT型;miRNA-499基因CC型SVR 74例,SVR%为71.2%,TT型SVR 19例,SVR%为51.2%,基因CC型SVR%高于基因TT型SVR%,差异有统计学意义(X^2=4.76,P<0.05);以miRNA-499基因CC型为对照,TT型OR值为2.954[90%CI(1.234,5.639)],差异有统计学意义(P<0.05)。结论miRNA-499(rs3746444)的SNPs可预测干扰素治疗丙肝的疗效,TT型获得较高SVR,对于临床治疗具有重要预测意义。

Objective To investigate the association between miRNA-499 gene polymorphism and interferon efficacy in patients with chronic hepatitis C. Methods 141 cases of patients with hepatitis C（ HCV） in our hospital were included and treated by subcutaneous 135 ~ 180μg PEG-IFN-α-2 a combined with 900 ~ 1200 mg/d Ribavirin,with a total course of 48 ~ 72 weeks.At the end of treatment,two-color fluorescent MGB probe of miRNA-499 gene was used to detect the polymorphism of rs3746444 site and according to the polymorphism of miRNA-499 gene,patients were divided into CC group,104 cases and TT group,37 cases,to explore the association of gene polymorphism with interferon efficacy. Results In 141 cases of HCV patients,there were 104 cases of CC genotype miRNA-499 gene and 37 cases of TT genotype,the frequency being 73. 6% and 26. 2%,respectively,with no CT genotype detected.74 cases of CC genotype miRNA-499 gene acquired sustained virologic response（ SVR）,with a SVR rate of 71. 2%and 19 cases of TT genotype acquired SVR,with a SVR rate of 51. 2%.The SVR rate of CC genotype was higher than TT genotype,the difference being statistically significant（ X^2= 4. 76,P〈0. 05）.CC genotype miRNA-499 gene as a contrast,the odds ratio（ OR） of TT genotype was 2. 954[90%CI（ 1. 234,5. 639） ],the difference being statistically significant（ P〈0. 05）.Conclusion Single nucleotide polymorphisms（ SNPs） of miRNA-499（ rs3746444） SNPs can predict the efficacy of interferon in the treatment of hepatitis C,and TT genotype acquired higher SVR,which has important predictive significance in clinical treatment.
