摘要
目的分析研究亚麻醉剂量氯胺酮-芬太尼辅助区域麻醉的应用。方法本研究对象为70例腹部或四肢手术患儿,其入院时间均在2015年8月~2017年1月期间,采用单双号分组法将所有患儿分为两组,对照组35例患儿接受氯胺酮麻醉,观察组35例患儿均给予亚麻醉剂量氯胺酮-芬太尼辅助椎管内麻醉,对两组患儿的生理指标进行比较,分析其麻醉效果、不良反应发生情况及生活质量。结果观察组患儿的心率与呼吸均明显较对照组低,其脉搏氧饱和度和平均动脉压均明显优于对照组,差异有统计学意义(P<0.05);观察组患儿的唤醒时间明显较对照组短,其氯胺酮用量以及氯胺酮追加次数均明显少于对照组,差异有统计学意义(P<0.05);观察组患儿的呼吸不畅发生概率、精神异常发生概率以及躁动发生概率均明显低于对照组,生活质量各项评分则明显较对照组高,差异有统计学意义(P<0.05)。结论亚麻醉剂量氯胺酮-芬太尼辅助区域麻醉的应用效果显著,且具有较高的安全性,可广泛应用于今后临床实践当中。
Objective To analyze the application of ketamine fentanyl assisted regional anesthesia to study the application of subcutaneous anesthesia ketamine-fentanyl adjunctive area anesthesia. Methods 70 children with abdominal or limb surgery whose admission time was from August 2015 to January 2017 were seleeted as the subjects of this study,and they were divided into two groups by single and even number grouping method.The control group (35 cases) was received ketamine anesthesia,and the observation group (35 cases) was received sub anesthetic ketamine ketamine fentanyl assisted spinal anesthesia.The physiological indexes of the two groups were compared,and the anesthetic effect,adverse reactions and quality of life were analyzed. Results The heart rate and respiration rate of the observation group were significantly lower than that of the control group,and the pulse oxygen saturation and mean arterial pressure were significantly better than those of the control group,P 〈 0.05;The wake-up time of the observation group was significantly shorter than that of the control group,and the dosage of ketamine and the times of ketamine addition were significantly less than those of the control group,P 〈 0.05;The incidence of respiratory distress,the probability of occurrence of mental disorders and the incidence of agitation in the observation group were significantly lower than those in the control group,and the scores of quality of life were significantly higher than those in the control group,P 〈 0.05. Conclusion The application of ketamine fentanyl combined with regional anesthesia is significant and safe,and can be widely used in clinical practice in the future
