2011-2015年佛山市禅城区活动性肺结核患者的登记与治疗管理分析

Analysis of registration and treatment management of active pulmonary tuberculosis patients in Chancheng District of Foshan City from 2011 to 2015

目的:回顾分析2011-2015年佛山市禅城区肺结核患者的发现登记和治疗管理情况,探索适合本地区活动性肺结核患者的发现方式和治疗管理模式。方法:采用描述流行病学的方法,对2011-2015年禅城区登记管理的活动性肺结核患者的发现方式、登记数据和治疗转归情况进行分析,并对社会效益进行评价。结果:全区共登记治疗管理活动性肺结核患者3 158例,其中新涂阳996例;发现方式以转诊和因症就诊的被动发现方式为主;新涂阳患者强化期末平均痰菌转阴率87.95%,治愈率90.46%;可避免9 343人感染结核菌,避免新发结核患者934人,为社会节约医疗费用63.65万元,为社会挽回损失总价值109.72亿元。结论:5年来禅城区结核病防治工作取得了较为显著的成效,较好地控制了我区结核病疫情,今后应广泛开展健康促进工作,多方面开展主动筛查,及时发现肺结核患者,并进行全程规范的系统管理,有效控制肺结核在人群中传播。

Objective：Through the analysis of registration and treatment management of active pulmonary tuberculosis patients in Chancheng District of Foshan City from 2011 to 2015,to explore the appropriate discovery mode and treatment management model for active pulmonary tuberculosis patients in this region.Methods：Using descriptive epidemiology,we analyzed the finding patterns,registration data and treatment outcomes of active pulmonary tuberculosis patients in Chancheng District from 2011 to 2015,and evaluated the social benefits.Results：A total of 3 158 cases of active pulmonary tuberculosis were registered and treated,including996 cases of new smear positive patients.The mode of discovery was mainly based on the passive discovery of referral and treatment for symptoms.The average sputum negative conversion rate in the late stage of smear positive patients was 87.95%,and the cure rate was 90.46%.These measures can avoid 9 343 people infected with tuberculosis,avoid 934 cases of new tuberculosis,can save the cost of medical treatment for the society about 636 thousand and 500 yuan,can recover the total value of the loss of10 billion 972 million yuan for society.Conclusion：In the past 5 years,tuberculosis prevention and control work in Chancheng District had achieved remarkable results,the epidemic situation of tuberculosis in our district had been well controlled.In the future,we should extensively carry out health promotion work,carry out various active screening,make timely detection of pulmonary tuberculosis patients,and take the whole process of standardized system management,so as to effectively control the spread of tuberculosis in the crowd.
