PDCA管理结合全程健康教育在环磷酰胺冲击治疗狼疮肾炎病人中的应用

Application of PDCA management combined with whole-course health education for patients with lupus nephritis treated by cyclophosphamide shock treatment

[目的]探讨PDCA管理结合全程健康教育在环磷酰胺冲击治疗狼疮肾炎病人中的应用。[方法]选取在本院应用环磷酰胺冲击治疗的80例狼疮性肾炎病人,根据随机数字表将病人分为观察组及对照组各40例,对照组治疗期间进行常规性随访管理,观察组治疗期间进行PDCA管理结合全程健康教育,随访时间为6个月。比较两组病人干预前后疾病管理能力、疾病知识评分及治疗效果。[结果]两组病人随访前疾病知识评分、自护行为评分比较差异无统计学意义(P>0.05),观察组随访3个月、6个月疾病知识评分、自护行为评分均高于对照组(P<0.05)。观察组随访3个月、6个月时血尿素氮(BUN)、肌酐(Scr)、24h尿蛋白定量较对照组均显著下降(P<0.05)。[结论]PDCA管理结合全程健康教育能有效提高环磷酰胺冲击治疗狼疮肾炎病人疾病管理能力及疾病知识水平(P<0.05),从而能有效改善病人肾功能,促进病人康复。

Objective：To probe into the application of PDCA management combined with whole-course health education for patients with lupus nephritis treated by cyclophosphamide shock treatment.Methods：A total of 80 patients with lupus nephritis treated by cyclophosphamide shock treatment in our hospital were divided into observation groups and control group according to the random number table,40 cases in each.The patients in control group received routine follow-up management during the treatment,while the patients in observation group received PDCA management combined with whole-course health education during the treatment over 6 months of follow-up.The disease management ability,disease knowledge score and treatment effect were compared between the two groups before and after intervention.Results：Before follow up,the disease knowledge score and self-care behavior scores of patients in two groups were not statistically significant（P〉0.05）,while the disease knowledge score and self-care behavior scores of patients in observation group were higher than those in control groups after follow up for 3 months and 6 months（P〈0.05）.The levels of blood urea nitrogen（BUN）,serum creatinine（Scr）,24 hurinary protein of patients in observation group were lower than those in control group after follow up for 3 months and 6 months（P 〈0.05）.Conclusions：PDCA management combined with wholecourse health education could improve disease management ability and disease knowledge level of patients with lupus nephritis treated by cyclophosphamide shock treatment（P〈0.05）,and it could effectively improve renal function and promote the rehabilitation of patients.
