社区护士家访服务对失智老人居家照顾者积极感受的影响研究

Study for the effects of community home visiting service on the positive aspects of care-giving among community-dwelling family of senile dementia patients

目的探究社区护士家访服务对失智老人居家照顾者积极感受的影响。方法选取上海市笔者所工作社区失智老人居家照顾者78例,随机分为干预组39例和对照组39例;对照组实施社区常规管理,干预组接受为期3个月的社区护士家访服务,包括失智症相关知识宣教、失智症日常生活照护、康复护理、居家安全防范、照顾者自我照顾指导等。采用照顾者积极感受问卷(Positive aspects of caregiving,PAC)进行干预前后评估。结果干预后,干预组积极感受得分明显高于对照组(t=5.811),差异有统计学意义(P<0.01)。结论社区护士家访服务能够为失智老人及居家照顾者提供社区医疗支持,提高照顾者应对失智症的信心,减轻疾病照护负担,提升照顾者的积极感受。

Objective To study the effects of community home visiting service on the positive feeling of caregiver among family caregivers of senile patients with dementia.Methods 78 family caregivers of senile dementia patients in community were randomly divided into two groups,39 were assigned in the intervention group and 39 were assigned in the control group.The control group was received routine community management,and the intervention group was received by community home visiting service,including the disease knowledge education,care skills for daily life,rehabilitation,home security of dementia patient and the self-care instruction for caregivers lasted three months.Family caregivers’ outcome was measured and evaluated by positive aspects of care-giving（PAC）inventory.Results After the intervention,the scores of PAC in the intervention group was significantly higher than that of the control group（t=5.811,P〈0.01）.Conclusion Community home visiting service provided the community health support for the dementia patients and their caregivers,enhance the confidence that respond to dementia,reduced the caregiver burden and improved the positive aspects of care-giving among family caregivers of dementia patients.
