中医体质类型与类风湿关节炎相关性

Correlation Analysis of Traditional Chinese Medicine Constitution Types and Rheumatoid Arthritis

目的 :探讨中医体质类型与类风湿关节炎的相关性。方法 :依据《中医体质分类与判定》标准设计调查问卷,对类风湿关节炎患者进行中医体质类型的调查,并运用统计学方法分析类风湿关节炎的易感中医体质类型。结果 :(1)RA单一体质易患体质为虚性体质,其中阳虚质最多。(2)RA患者不同中医体质在一般资料及临床表现方面存在差异。不同年龄段的RA患者在中医体质的分布上具有差异(P<0.05),在阴虚体质和血瘀体质中,60~70岁和大于70岁年龄段的患者较多;在气虚体质中,大于70岁的患者较少;在痰湿体质中,以小于50岁的患者为最多;而其他体质中,不同年龄段的患者体质分布相差不明显。不同体质在病程之间有显著差异(P=0.007),血瘀体质患者病程最长,为(10.4±8.43)年,其次为湿热质和阴虚质。(3)骨密度在RA患者不同体质类型中存在差异。阳虚体质患者骨密度数值最低,其次为血瘀体质、湿热体质和阴虚体质患者。结论 :中医体质与类风湿关节炎有明显相关性,在未来的防病治病中应更多的应用中医体质分型,以期更好地治疗疾病。

Objective ：To investigate the relationship between TCM constitution types and rheumatoid arthritis（RA）. Methods ：According to the standard of Classification and Judgment of TCM Constitution,a survey of TCM constitution of patients with rheumatoid arthritis were made and the types of TCM constitution that are susceptible to rheumatoid arthritis patients were analyzed by statistical method. Results ：（1）The patients with rheumatoid arthritis susceptible traditional Chinese medicine constitution is XU,and the number of patients with Yang deficiency is the most.（2）From the result,we could see the differences on general information and clinical manifestation. Among different constitutions,there existed different age distribution（P0.05）. There were differences between blood stasis and Yang deficiency,phlegm dampness,damp-heat,specific endowment. Among all the four age groups,Yang deficiency had the largest proportion;among the groups of age 60~70 and over 70 years,Yin deficiency and blood stasis had the larger proportion. In the group of over 70 years,Qi deficiency had the lower proportion. In the term of phlegm dampness,the group of less than 50 years had the largest proportion. There were significant differences in the course of disease（P=0.007）. The course of the disease was longer than the others,（10.4±8.43）years,followed by damp-heat and Yin deficiency.（3）There were significant differences among different constitutions on bone density. The bone density of Yang deficiency was the lowest,followed by blood stasis,damp-heat and Yin deficiency. Conclusion ：There were correlation between TCM constitution types and rheumatoid arthritis,so we should consider TMC constitution during prevention and treating rheumatoid arthritis.
