Practical exploration into home-based medical care, rehabilitation, and elderly care of Gulou Community Health Service Center in Qufu City

为破解"医养结合"的难题,改变以往医、养分离的传统服务模式,建立医疗机构和居家养老的融合通道,曲阜市鼓楼社区卫生服务中心对于居家"医康养"模式进行了探索,以家庭医生为切入点,整合养老机构、日间照料机构等社会资源,让老年人在家中得到连续、适宜、规范、便捷的养老服务和基本医护服务,实现老年群体基本医护服务和养老服务全覆盖新模式,并取得了阶段性成果。

To solve the difficulty of ＂combination of medical and elderly care＂, change the traditional service mode in which medical care is separated from elderly care, and establish an integrated channel between medical institutions and home-based elderly care, the Gulou Community Health Service Center in Qufu City explored the model of home-based ＂medical care, rehabilitation, and elderly care＂. By taking family doctor as the breakthrough point and integrating the social resources of elderly care and day care organizations, the center enabled elderly people to enjoy continuous, appropriate, standard, and convenient elderly care and basic medical care services, realized a new model covering both basic medical care and elderly care of the aged group, and made certain achievements in the current stage.
