应用快速康复措施促进直肠癌患者术后胃肠功能恢复的效果研究

Effect of rapid rehabilitation nursing care on recovery of patients with rectal cancer during perioperative period

目的探讨直肠癌保肛患者围手术期应用快速康复护理措施促进术后胃肠功能恢复的临床效果。方法选择2014年3月至2016年3月本院收治的60例直肠癌行保肛术患者为研究对象,随机分为观察组(n=30)和对照组(n=30)。对照组予以围手术期常规护理,观察组应用快速康复理念指导下的护理措施。比较两组术后首次肛门排气时间、首次进食时间;住院时间、住院费用,观察两组术后胃肠道不适症状的发生率。结果观察组首次肛门排气时间、首次进食时间为(45.62±10.31)h、(67.45±19.27)h,显著少于对照组,差异有统计学意义(P<0.05);观察组住院时间为(7.42±3.68)d,显著短于对照组的(12.91±4.22)d;住院费用为(3.52±0.93)万元,显著低于对照组的(5.86±1.34)万元,差异有统计学意义(P<0.05);观察组术后胃肠道不适症状发生率为6.67%,明显低于对照组的30.00%,差异有统计学意义(P<0.05)。结论快速康复理念指导下的护理措施可有效促进直肠癌保肛术患者术后胃肠功能恢复,并可减少胃肠道不适症状。

Objective To investigate the effect of rapid rehabilitation nursing care in patients with rectal cancer during perioperative period. Methods 60 patients of rectal cancer with anorectal diseases were recruited from March 2014 to March 2016 in our hospital as study subjects. The patients were randomly divided into treatment group（n = 30） and control group（n = 30）. All patients received operation for on rectal cancer. The control group was given routine nursing care in the perioperative period. The treatment group received the nursing measures under the concept of rapid rehabilitation. The anal exhaust time,time to first food intake,hospitalization time and hospitalization cost were compared between the two groups. The incidence of postoperative complications in the two groups was compared. Results Anal exhaust time and time to first food intake in the treatment group were（45.62±10.31） h and（67.45±19.27） h,respectively,which were significantly shorter than in control group（P〈0.05）. Hospitalization time in the treatment group was（7.42 ±3.68） d,which was significantly shorter than in the control group [（12.91 ±4.22） d]. Hospitalization expenses in the treatment group was（3.52±0.93） hundred thousand,which was significantly lower than in the control group [（5.86±1.34）] hundredthousand（P〈0.05）. The complication rate was 6.67% after operation in the observation group,which was significantly better than in the control group（30%,P〈0.05）. Conclusion Rapid rehabilitation nursing care could effectively improve recovery and lower risk postoperative discomfort in patients with rectal cancer during perioperative period.
