咪唑安定在内镜操作中的镇静作用及护理支持

The Sedative Effect of Dormicum in Endoscopic Manipulation and its Nursing Support

目的探讨咪唑安定(Dormicum)在内镜操作中的镇静作用及护理支持。方法 对40例接受内镜操作的患者,随机分为4组,运用咪唑安定后,观察镇静分级、遗忘程度、患者满意度及心率、血氧饱和度的变化。结果 与对照组相比,用药组患者的满意度明显提高(P<0.05)。第4组的镇静程度、遗忘程度明显优于第2、第3组(P<0.05)。对照组检查开始1 min血压显著升高(P<0.05),同时伴有心率显著加快(P<0.05)。第4组氧饱和度在用药后2 min～5 min有所下降(1％～2％),内镜操作开始后即回升。结论 应用咪唑安定0.05 mg/kg可显著改善内镜操作时患者的耐受性,具有较好的安全性,且护理支持中心电监护、供氧、建立静脉通路、镇静观察等至关重要。

Objective To investigate the sedative effect and nursing support of Dormicum in endoseopic manipulation . Method 40 cases were divided at random into 4 groups. The effect of sedation, the effect of amnesia , euphoria of patients, the heart rate, the arterial pressure and the saturation of oxygen were investigated. Results The patients were more euphoric in Dormicum treated groups than in control group ( P < 0.05 ) . The effect of sedation and amnesia was more satisfactory in group IV (0.05 mg/kg) than in group II (0.03 mg/ kg)and HI (0.04 mg/kg) ( P < 0.05) . In control group (group I ) , both MAP and heart rate were increased significantly in one minute after endoseopic manipulation ( P < 0.05) . In group IV , the saturation of oxygen was decreased (1% ~ 2% ) in 2-5 minutes after injection of Dormicum, but rose again after the beginning of endoseopic manipulation. Conclusion Dormicum at dosage of 0.05 mg/kg is safe and can ameliorate the tolerance of patients to endoscopy. The nursing supports such as electrocardiac monitor, oxygen support, venous circuit establishment, observation of the depth of sedation are imortant.
