DR联合支气管镜检查对肺结核与小叶性肺癌的临床诊断价值

Clinical Value of DR Combined with Bronchoscopy in the Diagnosis of Pulmonary Tuberculosis and Lobular Lung Cancer

目的探讨数字X射线机(DR)联合支气管镜检查诊断肺结核与小叶性肺癌的临床价值。方法分析150例疑似肺结核与小叶性肺癌患者的病例资料,疑似小叶性肺癌患者48例,疑似肺结核患者102例,以临床表现及手术病理活检结果为依据,比较观察常规检查和DR联合支气管镜检查的特异度、敏感度和准确度。结果 DR联合支气管镜检查诊断小叶性肺癌的灵敏度、特异度和准确度分别为74.19%、70.59%和72.92%,高于常规检查的51.61%、52.94%和52.08%(P<0.05);DR联合支气管镜检查诊断肺结核的灵敏度、特异度和准确度分别为71.79%、70.83%和71.57%,高于常规检查的55.13%、54.17%和54.90%(P<0.05)。结论 DR联合支气管镜检查对肺结核和小叶性肺癌的诊断效果好,有利于患者及早得到确诊和治疗,值得临床推广。

Objective To evaluate the clinical value of DR combined with bronchoscopy in the diagnosis of pulmonary tuberculosis and lobular lung cancer.Methods Clinical data of 150 cases of suspected pulmonary tuberculosis and lobular lung cancer patients were retrospectively analyzed,48 cases were suspected lobular lung cancer,102 cases were suspected pulmonary tuberculosis,according to the clinical manifestations and pathological biopsy results,the specificity,sensitivity and accuracy of routine examination and DR combined with bronchoscopy were compared.Results The sensitivity,specificity and accuracy of DR combined with bronchoscopy in the diagnosis of lobular lung cancer were 74.19%,70.59% and 72.92%,respectively,which were higher than those of routine examination（51.61%,52.94% and 52.08%）（P 0.05）.The sensitivity,specificity and accuracy of DR combined with bronchoscopy in the diagnosis of tuberculosis were 71.79%,70.83% and 71.57%,respectively,which were higher than those of routine examination（55.13%,54.17% and 54.90%）（P 0.05）.Conclusion DR combined with bronchoscopy for tuberculosis and lobular lung cancer has good diagnosis effect,it is conducive to early diagnosis and treatment of patients,and has clinical promotion value.
