The Effect of MR In-phase and Opposed-phase Sequence in the Diagnosis of Renal Cell Carcinoma

摘要 目的探讨磁共振(MR)同反相位序列对肾癌诊断的影响。方法采用回顾性研究方法,选择140例肾肿瘤患者作为研究对象,病理检查确诊为肾癌40例(恶性组)和肾良性肿瘤100例(良性组),都进行MR同反相位序列诊断与定量鉴别分析。结果所有患者的MR图像均显示清晰,无明显磁敏感伪影及图像变形。恶性组中有39例(97.5%)同相位序列表现为高信号,36例(90.0%)反相位序列表现为高信号。良性组中有78例(78.0%)同相位序列表现为高信号,72例(72.0%)反相位序列表现为高信号,两两对比都有统计学意义(P<0.05)。良性组患者的ADC值为(1.21±0.21)×10^(-3)mm^2/s,恶性组患者的ADC值为(1.69±0.30)×10^(-3)mm^2/s,两两对比有明显差异(P<0.05)。以1.355×10^(-3)mm^2/s为ADC阈值,MR同反相位序列诊断肾癌的敏感性和特异性分别为85.0%和92.1%。结论MR同反相位序列在肾癌诊断中的应用能提供临床定量诊断与鉴别依据,有很好的应用价值。 ObjectiveTo investigate the effect of magnetic resonance(MR)in-phase and opposed-phase sequence in the diagnosis of renal cell carcinoma.Methods 140 patients with renal tumors were selected as the research object,40 patients were renal cell carcinoma(malignant group)and 100 patients were benign renal tumors(benign group).All patients were analyzed with MR in-phase and opposed-phase sequence and quantitative differential diagnosis.Results The MR images of all patients were showed clear and there were no significant magnetic susceptibility artifacts and image distortion.In the malignant group,there were 39 patients(97.5%)were showed high signal in the in-phase sequence,and there were 36 patients(90.0%)were showed high signal in the opposed-phase sequence.In the benign group,78(78.0%)showed high signal in the same phase sequence,72 cases(72.0%)showed high signal sequence.The 2 groups had statistically significantly difference(P0.05).The ADC value of the benign group was(1.21±0.21)×10^(-3)mm^2/s,and the ADC value of the malignant group was(1.69±0.30)×10^(-3)mm^2/s,and there was significant difference compared between the 2 groups(P0.05).The sensitivity and specificity of MR in-phase and opposed-phase sequence in the diagnosis of renal carcinoma were 85.0%and 92.1%,respectively,with a threshold of 1.355×10^(-3)mm^2/s.Conclusion The application of MR in-phase and opposed-phase sequence in the diagnosis of renal cell carcinoma can provide a basis for clinical diagnosis and identification,and has a good application value.
