综合干预治疗尘肺的效果

Comprehensive Intervention for Pneumoconiosis

目的观察综合干预治疗尘肺患者的临床效果。方法将120例尘肺患者按治疗方法的不同分为肺泡灌洗组(灌洗组)、丹参药物治疗组(丹参组)、综合干预组(综合组),每组40例。灌洗组采用对症治疗+生理盐水(NS)大容量全肺灌洗(WLL);丹参组采取对症治疗+丹参片(4片·次-1,3次·d-1)口服;综合组对症治疗+丹参药物治疗+低体积比丹参液(每500mL NS加入丹参注射液4mL)全肺灌洗。观察各组患者治疗前、治疗1个月和治疗1年的肺功能[用力肺活量(FVC)、第1秒用力呼气量(FEV1)、一氧化碳弥散量(DLCO)]、动脉血氧饱和度(SaO2)、生活质量(CAT问卷评分)变化。结果治疗1个月后,3组患者肺功能、CAT评分、SaO2较治疗前均改善(P<0.05),综合组各项指标优于丹参组(P<0.05),与灌洗组比较差异无统计学意义(P>0.05);治疗1年后,综合组各项指标较其他2组改善明显(P<0.05)。结论尘肺患者采用综合干预治疗,1年内能够明显改善患者生活质量,延缓病变升级。

Objective To observe the clinical effect of comprehensive intervention on pneumoconiosis.Methods A total of 120 patients with pneumoconiosis were randomly divided into three groups,with 40 patients in each group.On the basis of symptomatic treatment,alveolar lavage group,Salvia miltiorrhiza treatment group and comprehensive intervention group were respectively given whole lung lavage(WLL)with normal saline(NS),oral Salvia miltiorrhiza tablets(4 tablets·time-1,3 times·day-1)and Salvia miltiorrhiza drug therapy+WLL with low concentration of Salvia miltiorrhiza solution(4 mL Salvia miltiorrhiza injection in 500 mL NS).Pulmonary function(FVC,FEV1,DLCO),arterial oxygen saturation(SaO2)and quality of life(CAT questionnaire score)were observed before and after treatment for 1 month and 1 year.Results After treatment for 1 month,lung function,CAT score and SaO2 were obviously improved in all the three groups(P<0.05).Furthermore,the indexes in comprehensive intervention group were superior to those in Salvia miltiorrhiza treatment group(P<0.05).However,there were no significant differences between comprehensive intervention group and alveolar lavage group(P>0.05).After treatment for 1 year,all indexes in comprehensive intervention group were significantly improved compared with alveolar lavage group and Salvia miltiorrhiza treatment group(P<0.05).Conclusion Comprehensive intervention can improve the quality of life and delay the progression of disease in patients with pneumoconiosis.
