摘要 目的探讨肠内营养支持在急性白血病患者化疗中的应用效果。方法将62例急性白血病患者按随机数字表法分为干预组(30例)和对照组(32例)。2组均采用常规化疗。在此基础上,干预组采用肠内营养支持治疗,对照组按平时患者饮食习惯进食。观察2组干预前、干预2周后营养指标[体质指数(BMI)、BMI差值,血清前白蛋白、前白蛋白差值]和输悬浮红细胞率、输机采浓缩血小板率及临床疗效、并发症(感染、黏膜炎)发生率。结果干预2周后,干预组BMI及血清前白蛋白水平均明显高于对照组,BMI差值、血清前白蛋白差值均明显低于对照组(P<0.05或P<0.01)。干预组感染、黏膜炎发生率均明显低于对照组(均P<0.05)。2组总有效率、输悬浮红细胞率、输机采浓缩血小板率比较差异无统计学意义(P>0.05)。结论对急性白血病化疗患者采用肠内营养支持,可以改善患者的营养状态,降低并发症发生率,但不能提高化疗缓解率。 Objective To investigate the efficacy of enteral nutritional support in acute leukemia patients treated with chemotherapy.Methods Sixty-two patients undergoing chemotherapy for acute leukemia were randomly assigned to receive either enteral nutritional support(intervention group,30 patients)or routine diet(control group,32 patients).Body mass index(BMI),BMI change,serum prealbumin,prealbumin change,suspended erythrocyte transfusion rate,concentrated machine-collectedplatelet transfusion rate,and incidence of complications(infection and mucositis)were observed before and after intervention for 2 weeks.Results Compared with control group,enteral nutritional support increased BMI and serum prealbumin level,and decreased changes in BMI and prealbumin and incidences of infection and mucositis(P<0.05 or P<0.01).There were no significant differences in total effective rate,suspended erythrocyte transfusion rate and concentrated machine-collectedplatelet transfusion rate between the two groups(P>0.05).Conclusion Enteral nutritional support can improve nutritional status and reduce incidence of complications,but can not increase remission rate of chemotherapy in patients with acute leukemia.
