不同方法治疗复杂胫骨平台骨折的疗效比较

Comparison of efficacy of different methods in the treatment of complex tibial plateau fractures

目的:对比支持钢板(sustaining plate,SP)、单侧锁定钢板(locking plate,LP)与双侧LP应用于复杂胫骨平台骨折病人的复位效果及膝功能恢复情况。方法:选择46例复杂型胫骨平台骨折病人作为研究对象,根据病人的CT检查结果,观察内侧髁有无冠状面骨折,将病人分为SP组(12例)、单侧LP组(14例)和双侧LP组(20例)。观察比较3组病人手术时间、术中出血量和术后愈合情况、膝关节功能、并发症发生情况。结果:SP组病人手术时间、术中出血量及愈合时间均明显高于其他2组病人(P<0.05~P<0.01);3组病人术后膝关节功能优良率和并发症发生率差异均无统计学意义(P>0.05)。结论:对于CT显示内侧髁有冠状位骨折的复杂型胫骨平台骨折病人,使用双侧LP术具有更好的疗效。

ObjectiveTo compare the reset effects and knee joint function of sustaining plate（ SP）,locking plate（LP） and doublelocking plate in treating complex tibial plateau fracture. Methods：The coronal fractures of internal condyle in 46 patients with complextibial plateau fracture were examined using CT. The patients were divided into the SP group （12 cases）,LP group （14 cases） andbilateral LP group（20 cases）. The operation time,bleeding loss,postoperative healing,knee jiont function and complications betweenthree groups were compared. Results：The operation time,bleeding loss and healing time in SP group were significantly higher than thosein other two groups（P 〈 0. 01 to P 〈 0. 05）. The differences of the excellent and good rate of postoperative knee joint function andincidence rate of complications between three groups were not statistically significant （ P 〉 0. 05 ）. Conclusions： The effect of thebilateral LP in treating the coronal fractures of internal condyle in patients with complex tibial plateau fractures is better.
