灌肠方结合美沙拉嗪治疗溃疡性结肠炎的临床疗效研究

Clinical effect of enema combined with mesalazine in the treatment of ulcerative colitis

目的探讨中药灌肠方结合美沙拉嗪治疗溃疡性结肠炎的临床疗效。方法选择94例溃疡性结肠炎患者为观察对象,按照不同药物治疗分为观察组与对照组,每组47例。对照组给予患者口服使用美沙拉嗪片治疗,观察组在对照组基础上结合使用中药灌肠方治疗。两组治疗周期均为2周。观察治疗后临床疗效、C-反应蛋白(CRP)、结肠炎活动指数(UCAI)、血沉(ESR)以及临床症状情况。结果观察组总有效率为97.87%,对照组为85.11%,组间比较差异有统计学意义(χ2=37.22,P<0.05);两组治疗后CRP、UCAI及ESR水平均降低,组内比较有统计学差异(t分别=4.64、4.64、5.22;3.89、4.15、4.10,P均<0.05);且观察组治疗后各指标较对照组降低更为明显(t分别=4.77、5.10、5.18,P均<0.05)。观察组腹泻、发热、腹痛、脓血便以及里急后重等症状积分均明显低于对照组(t分别=4.03、5.53、5.00、4.63、4.92,P均<0.05)。结论中药灌肠方结合美沙拉嗪治疗溃疡性结肠炎能降低CRP、UCAI及ESR水平,临床疗效明显。

Objective To investigate the clinical efficacy of enema combined with mesalazine in the treatment of ulcerative colitis. Methods Totally 94 patients with ulcerative colitis were selected as observation objects and divided into observation group and control group with 47 cases in each.The control group was given oral administration of mesalazine,and the observation group was treated with enema on the basis of the control group.The courses of two groups were 2 weeks.The clinical efficacy,C-reactive protein（CRP）,ulcerative colitis activity index（UCAI）,erythrocyte sedimentation rate（ESR）and clinical symptoms of two groups were observed after treatment. Results The total effective rate was 97.87% in the observation group and 85.11% in the control group. The difference between the two groups was statistically significant（χ~2=37.22,P〈0.05）. The levels of CRP,UCAI and ESR after treatment were all significantly decreased in the two groups when compared to before treatment,and there was a statistically significant difference between the two groups（t=4.64,4.64,5.22;3.89,4.15,4.10,P〈0.05）. The levels of CRP,UCAI and ESR of the observation group after treatment were significantly lower than those of the control group（t=4.77,5.10,5.18,P〈0.05）.The diarrhea,fever,abdominal pain,pus and blood stool and tenesmus symptom scores of observation group were lower than the control group（t=4.03,5.53,5.00,4.63,4.92,P〈0.05）. Conclusion Traditional Chinese medicine enema combined with mesalazine in the treatment of ulcerative colitis can obviously reduce the level of CRP,UCAI and ESR.
