摘要 目的:比较正电子发射型计算机断层显像(PET)/电子计算机断层扫描(CT)与增强CT对肺癌淋巴结及其分期的诊断价值。方法:选取80例(560枚病灶)肺癌患者,均行PET/CT和CT增强扫描检查,以病理检查结果作为金标准,比较PET/CT与增强CT的阳性预测值、阴性预测值、特异度、灵敏度及分期诊断准确率。结果:PET/CT的阳性预测值、阴性预测值、特异度及灵敏度均明显高于增强CT,差异有统计学意义(P<0.05);PET/CT与增强CT对N0期淋巴结的诊断准确率比较差异无统计学意义(P>0.05),PET/CT对N1和N2期淋巴结的诊断准确率明显高于增强CT,差异有统计学意义(P>0.05)。结论:与增强CT扫描比较,PET/CT检查技术在诊断肺癌及淋巴结分期方面效果更好,可作为临床诊断的重要依据。 Objective: To compare diagnostic values of positron emission computed tomography( PET)/computed technology( CT) and enhanced CT in diagnosis and staging of lung cancer lymph node metastasis. Methods: Selection of 80 patients with lung cancer( 560 lesions) were retrospectively analyzed. All patients underwent PET/CT and CT enhanced scan,and the pathological results were taken as the gold standard. The positive predictive values,negative predictive values,specificities,sensitivities and staging diagnosis rates of PET/CT and enhanced CT were compared. Results: The positive predictive value,negative predictive value,specificity and sensitivity of PET/CT were significantly higher than those of enhanced CT,and the differences were statistically significant( P〈0.05). Further,the difference of the diagnostic accuracy of PET/CT and enhanced CT in the diagnosis of N0 lymph node was not statistically significant( P 〉0. 05); however,that of the diagnosis rate of N1 and N3 lymph nodes was statistically significant( P〉 0.05). Conclusions: Compared with enhanced CT scan,PET/CT examination in the diagnosis of lung cancer and lymph node staging is more effective and can be used as an important evidence for clinical diagnosis.
