甲状腺囊腺癌的超声征象总结及误诊分析

摘要 目的探讨甲状腺囊腺癌的超声征像特征并对误诊原因进行分析。方法对经病理证实的23例甲状腺囊腺癌的超声表现进行回顾性分析。结果 23例甲状腺囊腺癌,实性为主型14例(漏诊2例),囊性为主型9例(漏诊3例),漏诊率分别为14.2%、33.3%;直径>2 cm(69.6%);结节内部结构主要表现为:偏心锐角型15例(65.2%);分叶或锯齿状21例(91.3%);低回声18例(78.2%);微钙化14例(60.8%)。结论超声影像特点对甲状腺囊腺癌的诊断具有重要的临床价值,可减少甲状腺囊腺癌的误诊率。 Objective To investigate the ultrasonographic features of thyroid cystadenocarcinoma and misdiagnosis analysis. Methods The ultrasonographic manifestations of 23 cases with thyroid cystadenocarcinoma confirmed by pathology were retrospectively analyzed. Results 14 were mainly solid types( 2 cases with misdiagnosis) and 9 were cystic types( 3 cases with misdiagnosis). The misdiagnosis rates were 14.2% and 33.3%,respectively.The diameter was〉2 cm( 69.6%).The internal structure of the nodules mainly showed as follows: 15 cases( 65.2%) with acute eccentric angle,21 cases( 91.3%) with lobulated or jagged,and 18 cases( 78.2%) with low echo and 14 cases( 60.8%) with microcalcification.Conclusions The ultrasonographic features have important clinical value in diagnosis of thyroid cystadenocarcinoma,which can reduce the misdiagnosis rate of thyroid cystadenocarcinoma.
