血清视黄醇结合蛋白4联合C-反应蛋白诊断妊娠期早期糖尿病的价值

Value of serum retinol binding protein 4 combined with C-reactive protein in early diagnosis of gestational diabetes mellitus

目的对血清视黄醇结合蛋白4联合C-反应蛋白用于妊娠期早期糖尿病诊断中的价值予以分析,为其临床诊断提供参考。方法采用对照研究对血清视黄醇结合蛋白4联合C-反应蛋白用于妊娠期早期糖尿病诊断的价值进行验证。选取2015年7月-2016年7月北京儿童医院顺义妇儿医院诊治的妊娠期早期糖尿病疑似病例111例为研究对象,将其随机分为观察组和对照组。观察组根据血清视黄醇结合蛋白4联合C-反应蛋白检查结果进行诊断;对照组根据血清视黄醇结合蛋白4检测结果进行诊断。以血糖检测诊断结果为标准进行组间比较,并对两组诊断的准确性进行评价。结果观察组和对照组诊断准确率分别为89.3%和78.2%,观察组显著高于对照组,差异有统计学意义(P<0.05);两组诊断敏感性分别为75.0%和69.1%,观察组显著高于对照组,差异有统计学意义(P<0.05);两组患者的总依从率分别为92.9%和94.5%,差异无统计学意义(P>0.05)。结论血清视黄醇结合蛋白4联合C-反应蛋白用于妊娠期早期糖尿病诊断中具有准确性高,敏感性高,依从性高等优势。

Objective To analyze the value of serum retinol binding protein 4 （RBP4） combined with C-reactive protein （CRP） in early diagnosis of gestational diabetes mellitus （GDM）, provide reference for clinical diagnosis. Methods A ease-control study was con- ducted to verify the value of RBP4 combined with CRP in early diagnosis of GDM. A total of 111 eases suspected of early GDM, diagnosed and treated in the hospital from July 2015 to July 2016 were selected and randomly divided into observation group and control group. The cases in observation group were diagnosed according to the detection ：esuhs of RBP4 combined with CRP, while the cases in control group were diagnosed according to the detection results of RBP4. A compariscn among groups was conducted taking the diagnostic results of blood glu- cose as gold standard, the diagnostic accuracies in the two groups were evaluated. Results The diagnostic accuracy in observation group was 89. 3 %, which was statistically significantly higher than that in control group （78.2%） （P〈0.05） . The diagnostic sensitivity in ob- servation group was 75. 0% , which was slatistically significantly higher than that in control group （69.1% ） （P〈0. 05） . The total compliance rates in the two groups were 91.2% and 94. 5% , respectively, there was no statistically significant difference between the two groups （P〉0. 05 ） . Conclusion RBP4 combined with CRP has the advantages of high accuracy, high sensitivity, and high compliance in early diagnosis of GDM.
