DSA引导聚桂醇泡沫硬化剂治疗下肢静脉曲张的效果及对患者血清TNF-α、IL-6的影响

Influence of DSA-guided polyol foam sclerotherapy on varicose veins of lower extremity and its influence on serum TNF-α and IL-6 levels

目的探讨DSA引导聚桂醇泡沫硬化剂治疗下肢静脉曲张的效果及对患者血清肿瘤坏死因子(TNF-α)、白介素-6(IL-6)的影响。方法选取2012年7月~2016年6月因下肢静脉曲张于我院行手术治疗的89例患者作为研究对象,随机分为对照组(44例)和观察组(45例)。对照组采用常规剥脱术,观察组采用DSA引导下聚桂醇泡沫硬化剂治疗,比较两组的手术疗效。结果观察组的住院天数、手术时间短于对照组,术中出血量、住院费用少于对照组(P<0.05);观察组治疗24 h的TNF-α、IL-6水平低于对照组,差异有统计学意义(P<0.05);观察组的皮肤麻木感、皮下瘀血发生率低于对照组,且总发生率更低,差异有统计学意义(P<0.05);观察组的1年内复发率为13.33%,低于对照组的34.09%(P<0.05)。结论 DSA引导聚桂醇泡沫硬化剂治疗下肢静脉曲张效果显著,具有创伤小且并发症发生率低的优势,值得推广。

Objective To investigate the influence of DSA-guided polyol foam sclerotherapy on varicose veins of lower extremity and its influence on serum tumor necrosis factor （TNF-α and interleukin-6 （IL-6） levels in patients with lower extremity varicose veins.Methods Altogether 89 cases of patients with varicose vein of lower extremity who were treated in our hospital with surgeries from July 2012 to June 2016 were enrolled and randomly divided into the control group （44 cases） and the observation group （45 cases）.The control group was treated with conventional stripping operation,and the observation group was treated with DSA-guided polyol foam selerotherapy.The curative effect was compared be- tween the two groups.Results The hospitalization time,intraoperative bleeding volume,operation time and hospitalization costs of the observation group were less than those of the control group （P〈0.05）.The level of TNF-α and IL-6 of the observation group at 24 h treatment were lower than those of the control group,with statistically significant difference （P〈0.05）.The skin numbness and subcutaneous bleeding incidence rate of the observation group were lower than those of the control group,and the total incidence rate was also lower,with statistically significant difference （P〈0.05）.The 1-year recurrence rate of the observation group was 13.33%,which was lower than 34.09% of the control group （P〈0.05）.Con- elusion The DSA-guided polyol foam selerotherapy has significant effect in the treatment of lower extremity varicose veins,with less trauma and low incidence of complications,which is worth promoting.
