温柔分娩理念管理模式在初产妇孕产期中的应用及对其认知水平和分娩结局的影响

The application of management model of tender childbirth concept in primipara pregnancy and the influence on the cognitive level and childbirth outcome

目的:探讨温柔分娩理念管理模式在初产妇孕产期中的应用及对其认知水平和分娩结局的影响。方法:选取2016年1~12月我院产科门诊建卡并接受温柔分娩理念服务的初产妇150名作为观察组;选择同期接受常规孕产期保健服务的初产妇150名作为对照组,对比两组初产妇情绪状态、检查治疗依从性、Mulleetr阵痛评分、分娩认知评分、分娩结局、总产程及产后2 h出血量等。结果:观察组初产妇干预后焦虑抑郁评分均低于对照组(P<0.05);观察组初产妇检查治疗依从性与分娩认知水平评分高于对照组(P<0.05),Mulleetr阵痛评分明显低于对照组(P<0.05);观察组初产妇剖宫产率低于对照组(P<0.05),总产程短于对照组(P<0.05),产后2 h出血量明显少于对照组(P<0.05);观察组新生儿第1次胎便时间与对照组相比明显较短(P<0.05),第3天体质量下降少于对照组(P<0.05),胆红素水平明显低于对照组(P<0.05)。结论:温柔分娩理念管理模式有利于改善初产妇孕产期心理状态与检查治疗行为,提高认知水平与疼痛耐受能力,缩短产程,并能够降低剖宫产率,减少产后2 h出血量,改善新生儿适应性。

Objective: To explore the application of management model of tender childbirth concept in primipara pregnancy and the influence on the cognitive level and childbirth outcome. Methods: A total of 150 primiparas who built cards and received tender childbirth concept service at the obstetrics clinic of our hospital from January to December 2016 were selected as the observation group; a total of 150 primiparas who received conventional prenatal care service over the corresponding period were selected as the control group. The emotional state,examination and treatment compliance,Mulleetr labor pains score,childbirth cognitive score,outcome of delivery,total stage of labor and 2 h postpartum hemorrhage volume of primiparas were compared between the two groups. Results: After intervention,the anxiety and depression scores of primiparas in the observation group were lower than those in the control group( P < 0. 05); examination and treatment compliance and childbirth cognitive score in the observation group were higher than those in the control group( P <0. 05),and the Mulleetr labor pains score was significantly lower than that in the control group( P < 0. 05); the cesarean section rate in the observation group was lower than that in the control group( P < 0. 05),the total stage of labor was shorter than that in the control group( P < 0. 05),and the 2 h postpartum hemorrhage volume was significantly less than that in the control group( P < 0. 05); the first neonatal meconium time in the observation group was significantly shorter than that in the control group( P < 0. 05),the body mass decline on the third day after childbirth was less than that in the control group( P< 0. 05),and the serum bilirubin level was significantly less than that in the control group( P < 0. 05). Conclusion: The management model of tender childbirth concept is conducive to improving psychological status and examination and treatment behaviors of primiparas,enhancing cognitive level and pain tolerance and shortening total stage of labor,reducing cesarean section rate and 2 h postpartum hemorrhage volume and improving neonatal adaptability.
