摘要
目的:探讨层级护理管理模式对白血病患者经外周静脉置入中心静脉导管(PICC)后导管相关感染的影响。方法:选取2016年3月~2017年2月我院收治的84例置入PICC的白血病患者作为研究对象,将所有患者随机等分为观察组和对照组,观察组采用层级护理管理模式,对照组采用传统护理。比较两组患者导管相关感染的发生率及满意度,并分析PICC导管相关感染的相关因素。结果:观察组患者导管相关感染发生情况明显低于对照组(P<0.05)。观察组患者对护理工作的满意度明显高于对照组(P<0.05)。患者性别、年龄、穿刺部位及疾病与PICC导管相关感染无显著相关性(P>0.05)。Hb<60 g/L及置管时间>180 d与PICC导管相关感染呈正相关(P<0.05),层级护理管理与PICC导管相关感染呈负相关(P<0.05)。结论:层级护理管理能有效降低白血病患者PICC导管相关感染的发生率,提高患者满意度。
Objective: To explore the effect of hierarchical nursing management model on catheter-related infections after peripherally inserted central venous catheter( PICC) in leukemia patients. Methods: A total of 84 leukemia patients with PICC admitted to our hospital from March 2016 to February 2017 were selected as the objects for study. All the patients were divided into observation group and control group. The observation group was treated with hierarchical nursing management model,and the control group received traditional nursing. The incidence of catheter-related infections were compared between patients in the two groups,and the related factors of PICC catheter-related infections were analyzed. Results: The incidence of catheter-related infections in the observation group was significantly lower than that in the control group( P < 0. 05). The nursing satisfaction of the patients in the observation group was significantly higher than that in the control group( P < 0. 05). There was no significant correlation between the sex,age,puncture site and disease of the patients with PICC catheter-related infections( P > 0. 05). Hb < 60 g/L and catheterization time > 180 d were positively correlated with PICC catheter-related infections( P < 0. 05),and hierarchical nursing management was negatively correlated with PICC catheter-related infections( P < 0. 05).Conclusion: The hierarchical nursing management can effectively reduce the incidence of PICC catheter-related infections in leukemia patients and improve the patient satisfaction.
