超声检查在神经纤维瘤病中的意义分析

Analysis of the significance of ultrasonography in neurofibromatosis

目的:探讨神经纤维瘤病的彩色多普勒超声表现,以提高诊断的准确性。方法:回顾性分析13例被临床或手术病理诊断为神经纤维瘤病的患者临床表现及超声图像特点。结果:神经纤维瘤病以多发结节型居多,声像表现为沿神经走向分布、大小不一的类圆形、分叶状或梭形低回声结节,结节一端或两端与神经干相连;其次丛状型也较多,声像为沿神经干长轴蔓延生长的低或等回声结节,呈螺旋状增粗扭曲;弥漫型的较少,易误诊,特征性声像为皮下脂肪层增厚,伴有弥漫性高回声背景中穿行着条索状低回声肿物。超声提示诊断9例(69%,9/13)、误诊4例(31%,4/13)。结论:根据超声图像特点及患者的临床症状及体征,超声检查能够诊断神经纤维瘤病,为临床诊断提供客观、明确的依据,并且对神经纤维瘤病患者术后是否复发、恶变进行随访有重要意义。

Objective To study the color Doppler ultrasonography of neurofibromatosis to improve the accuracy of diagnosis. Method The clinical manifestations and ultrasonographic features of 13 patients with clinical or surgical pathologic diagnosis of neurofibromatosis were retrospectively analyzed. Results The results of neurofibromatosis to multiple nodular majority,the visual performance of the distribution along the nerve,the size of different round,lobulated or fusiform hypoechoic nodules,nodules at one or both ends connected with the nerve; Followed by more plexiform type,the sound like the long axis of the nerve along the spread of the growth of low or equal echo nodules,spiral twisted twisted; diffuse type less,easy to misdiagnosis,the characteristics of subcutaneous fat Layer thickening,accompanied by diffuse high echo background through the line-like hypoechoic mass. Ultrasound prompted the diagnosis of 9 cases（69%,9/13）,misdiagnosis in 4 cases（31%,4/13）. Conclusion Ultrasonography can diagnose neurofibromatosis according to the characteristics of ultrasound image and the clinical symptoms and signs of patients. It can provide objective and definite basis for clinical diagnosis,and it is important to follow the recurrence and malignant transformation of patients with neurofibromatosis.
