慢性疼痛门诊患者心理体验的现象学研究

A phenomenological study on psychological experience of outpatients with chronic pain

目的了解慢性疼痛门诊患者的心理体验,为从心理学视角管理慢性疼痛提供参考。方法采用目的抽样法对15例慢性疼痛门诊患者进行半结构式深度访谈,采用Giorgi的现象学分析法进行资料分析。结果共提炼出6个主题:疼痛缓解需求,对疼痛的灾难化认知,自我感受负担,存在疾病不确定感,恐动和社交回避心理,对疼痛的无奈接受。结论慢性疼痛患者存在复杂的心理活动,需重视患者的心理体验,采取有效措施干预负性心理,使患者积极应对疼痛。

Objective To describe psychological experience of outpatients with chronic pain and to provide reference for pain management through psychological intervention. Methods Fifteen patients with chronic pain attending a pain clinic were selected using purposive sampling and were invited to participate in an in-depth, semi-structured interview to describe their experience of pain, then the interviews were transcribed and analyzed using Giorgi＇s phenomenological steps. Results Six themes were identified, including expectations of pain control, perceptions of pain eatastrophizing, self-perceived burden, presence of illness uncertainty, kinesiophobia and social avoidance, and resigned acceptance of pain. Conclusion Patients with chronic pain do have complicated psychological experiences. These findings suggested that medical personnel should pay attention to psychological experience of patients, take effective measures to alleviate their negative feelings, thus to help them cope with chronic pain positively.
