27例踝关节旋前外旋Ⅲ～Ⅳ度骨折的外科治疗观察

Surgical Treatment of 27 Grade Ⅲ or Ⅳ Pronation-external Rotation Ankle Fractures

目的:探讨踝关节旋前外旋型Ⅲ~Ⅳ度骨折的手术治疗效果。方法:回顾性分析2009-2016年笔者所在科采用手术治疗的27例旋前外旋踝关节损伤的临床特点及治疗效果,全部病例均采用切开复位内固定手术。结果:所有的27例病例均得到随访,随访时间12~16个月,平均13.9个月。按照踝关节Baird-Jackson评分标准,术后优22例,良4例,中1例。结论:踝关节旋前外旋Ⅲ~Ⅳ度损伤有明显的手术适应证,通过外科手术进行解剖复位及坚强的内固定可取得满意的疗效。

Objective：To observe the clinical outcomes of surgical treatment of 27 grade Ⅲ or Ⅳ pronation-external rotation ankle fractures.Method：The clinical characteristics and therapeutic effects of 27 cases of pronation and supination ankle injuries from 2009 to 2016 in our department were retrospectively analyzed.All patients were treated by opened reduction and internal fixation and internal fixation.Result：All of the cases were followed up for 13.9 months in average（ranged from 12 to 16 months） and had healed well,there were no infection cases.According to the ankle Baird-Jackson score criteria,the result was excellent in 22 cases,good in 4 cases,and fair in 1 case.Conclusion：There are obvious indications of surgery in grade Ⅲ and Ⅳ ankle fractures,the satisfactory functions after surgery are related with anatomic reduction and stable internal fixation closely.
