摘要
目的:通过脐血血气分析,探讨其与新生儿窒息、缺氧的关系。方法:对70例足月产新生儿,出生后马上采集脐动脉血并进行血气分析,与Apgar评分诊断窒息情况进行比较分析。结果:与对照组对比,窒息组患者pH值、PaO_2更低,PCO_2更高,差异有统计学意义(P<0.05)。与4~7分窒息组新生儿相比,窒息组0~3分新生儿的pH值、PaO_2更低,PCO_2更高,差异有统计学意义(P<0.05)。对照组8例(17.02%)新生儿发生酸中毒,窒息组17例(73.91%)新生儿发生酸中毒,窒息组显著高于对照组,差异有统计学意义(P<0.05)。结论:生后脐血的血气分析结果能够客观地反映胎儿缺血缺氧程度,其可在生化水平反映新生儿的代谢状态,对新生儿窒息诊断更有提示作用。
Objective:To observe the significance of neonatal cord blood gas analysis in neonatal asphyxia. Method:70 cases of full-term newborn in our hospital were selected,umbilical cord blood was collected immediately after birth, and blood gas analysis was performed; the Apgar score was used to compare the diagnosis of asphyxia.Result:Compared with the normal group,pH value,PaO_2 and PCO_2 were higher in asphyxia group,the differences were statistically significant(P〈0.05).Compared with the neonatus with 4~7 score in the neonatal asphyxia group,the pH value,PaO_2 and PCO_2 of neonatus with 0~3 Apgar score in neonatal asphyxia group were higher,the differences were statistically significant(P〈0.05).In the control group,8 cases of neonatal lactic acid increased,the incidence of acidosis was 17.02%;in observation group,17 cases of neonatal lactic acid increased,the incidence was 73.91%,the indexes in observation group were significantly higher than those in the control group,the differences were statistically significant(P〈0.05).Conclusion:The results of blood gas analysis of umbilical cord blood can objectively reflect the degree of fetal hypoxia and ischemia,reflect the metabolic status of newborns in biochemical level.It is suggestive of the diagnosis neonatal asphyxia.
