摘要
目的探讨Avaulta盆底重建术的临床效果。方法回顾性分析2013年12月至2015年12月在武汉大学人民医院行Avaulta盆底重建术的患者212例的临床资料,通过盆底功能障碍性疾病症状问卷(PFDI)-20和盆底疾病生活质量影响问卷短表(PFIQ)-7评估患者手术前及术后1个月、6个月后生活质量的改善,评估手术治疗效果,并通过随访统计手术并发症及复发例数。结果 POP-QⅢ期157例,POP-QⅣ期55例。术前、术后1个月、术后6个月PFIQ-7评分分别为(79.2±10.5)分、(25.1±7.6)分、(2.5±1.1)分,PFDI-20评分分别为(65.5±12.4)分、(10.4±4.9)分、(3.0±0.8)分,术后1个月、6个月PFIQ-7、PFDI-20评分与术前比较差异有统计学意义。泌尿系损伤3例,尿潴留20例,血肿4例,复发2例,未见网片侵蚀、网片暴露发生,无明显慢性盆腔痛发生。结论 Avaulta盆底重建术能明显改善患者的生活质量,但需警惕并发症的发生。
Objective To research the clinical efficacy of Avaulta pelvic floor reconstruction. Methods Retrospectively analyze the clinical data of patients receiving Avaulta pelvic floor reconstruction from December 2013 to December2013 in our hospital,and evaluate improvement in quality of life through the PFIQ-7 and PFDI-20 questionnaires before surgery,one month after suegery and six months after suegery in order to evaluate the surgical treatment effect;the complications and recurrence cases were recorded by the follow-up.Results 157 cases of POP-Q Ⅲ degree and 55 cases of POP-Q Ⅳ degree were reported. The score of PFIQ-7 before surgery,one month after surgery and six months after surgery was 79.2±10.5,25.1±7.6 and 2.5±1.1.The score of PFDI-20 before surgery,one month after surgery and six months after surgery was 65.5±12.4,10.4±4.9 and 3.0±0.8.Comparing the scores of PFIQ-7 and PFDI-20 questionnaires before surgery with those one month after surgery and six months after sugery,the difference were statistically significant. There were 3 cases of urinary tract injury,20 cases of urinary retention,4 cases of hematoma and 2 cases of recurrence. No mesh eruptive or mesh exposure occurred,and no obvious chronic pelvic pain occurred.Conclusion Avaulta pelvic floor reconstructio can significantly improve the quality of life,but we need to be alert to the occurrence of complications.
