完整结肠系膜切除术联合术后FOLFOX4治疗Ⅲ期结肠癌患者临床效果的研究

Clinical effect of complete mesocolic excision combined with postoperative FOLFOX4 in the treatment of stage Ⅲ colon cancer

目的探讨完整结肠系膜切除术(CME)联合术后FOLFOX4对Ⅲ期结肠癌临床治疗效果。方法选取64例Ⅲ期结肠癌患者,根据入院顺序随机分为研究组和对照组,每组各32例。对照组患者实施CME进行治疗;研究组在对照组手术治疗基础上实施FOLFOX4治疗,2周为一周期,连续治疗4个周期。分别统计分析两组患者完全缓解(CR)、部分缓解(PR)、稳定(NC)、进展(PD)治疗效果和生存情况。结果与对照组相比,研究组CR患者显著增加(P<0.01),PR患者差异无统计学意义(P>0.05),研究组近期客观总有效率(87.50%)显著高于对照组(59.38%),差异有高度统计学意义(P<0.01);研究组复发、转移和死亡情况均低于对照组患者,差异有统计学意义(P<0.01,P<0.05)。结论 CME联合术后FOLFOX4治疗Ⅲ期结肠癌能够显著增加临床治疗效果,降低复发、转移和死亡率,临床意义显著。

Objective To investigate the clinical effect of complete mesocolic excision（CME） combined with post-operative FOLFOX4 in the treatment of stage Ⅲ colon cancer.Methods 64 patients with stage Ⅲ colon cancer in our hospital were randomly divided into the study group and the control group according to their admission order, with 32 cases in each group.Patients in the control group were treated with CME while the study group was treated with FOLFOX4 and CME.2 weeks a cycle and 4 cycles in a roll.The treatment effect（CR, PR, NC, PD） and survival of patients in the two groups were analyzed and contrasted.Results Compared with the control group, patients with CR in the study group significantly increased（P 〈 0.01） and there was no significant difference between patients with PR （P 〉 0.05）.The objective efficiency of the study group（87.50%） was significantly higher than that of the control group （59.38 %） and the difference was statistically significant （P 〈 0.01 ）. The recurrence, metastasis and death of the study group were lower than those of the control group and the difference was signifieant（P 〈 0.01, P 〈 0.05）.Conclusion The CME combined with postoperative FOLFOX4 treatment could significantly increase the clinical curative effect of Stage Ⅲ colon cancer, reduce recurrence, metastasis and mortality rate.
