摘要
医学教育是一个连续的统一体,不论是大学中的实习强化教育还是毕业后的住院医师规范化培训教育,最终目标是应用所学理论知识结合临床实践经验,提高临床思维能力,成为合格临床医生。将儿科实习带教与住院医师规范化培训并轨培训的模式,是一种新型的培养模式,充分利用医院规培的师资与硬件资源,实现实习带教内容与国家住院医师规范化培训、考核的接轨,强化对实习生实践能力的训练,使临床实习带教有标准,质量有监控,临床实践循序渐进,是提高医院医疗水平和发展临床医学的科技战略措施。我院是新疆兵团系统住院医师规范化培训基地,同时也是石河子大学的附属医院,每年招收来自不同医学院校的规培学员及实习学生,而我科作为儿科教研室及专业培训基地,在不断探索和实践中,将实习学生与规培学员有机会结合在一起,因地制宜地探索了一套培训流程,进行系统而规范的培训,包括:(1)强化入科培训,转变意识,转换角色;(2)明确培养目标,制定个体化培训计划;(3)优化教学内容,提高临床思维的多种教学模式(诊治患者的临床观摩;病例导入式培训方式的应用;技能操作的培训;疑难病例讨论;主任查房;业务学习等);(4)构建高素质的师资队伍,完善教学体系;(5)构建岗位胜任力考核体系,提高带教质量;(6)沟通交流,师生座谈。通过以上等措施,将儿科住院医师培训与实习带教并轨培训,不断开拓新思路,提高我院临床实习学生的规范化培训质量及标准,使医学生的在校教育上一台阶,提高医学教育的整体水平。
Medical education is a continuous unity, for both the practice enhancement education in the university and graduate resident standardization training, the ultimate goal is to apply the theoretical knowledge combined with clinical practice experience, to improve the ability of clinical thinking, to become a qualified clinician. It is a new kind of training mode of combination training mode of pediatric internship teaching and resident standardization training, it makes full utilize of teachers and the hardware resources in hospitals, implements the combination of practice teaching content, resident standardization training and evaluation, strengthens the practice ability training for interns, the clinical practice teaching has standards, the quality is monitored, and the clinical practice is step by step, it is technology strategic measure to improve the medical level of hospitals and the development of clinical medicine. Our hospital is the base of resident standardization training for Xinjiang Production and Construction Corps System, it is also the affiliated hospital of Shihezi University, it enrolls standardization training students and intern students from different medical schools every year, and as a pediatric teaching and research section and professional training base, in the constant exploration and practice, our department unites the standardization training students and intern students, develops a set of training process adjust to local conditions, carries out systematic and standardized training, including:(1) strengthening the entrance training, changing consciousness, transforming roles;(2) clearing training objectives, setting individualized training programs;(3) various teaching modes to optimize the teaching content and improve the clinical thinking( such as imitate study on diagnosis and treatment; application of case-based integrated teaching; operation skills training; difficult cases discussion, ward rounds, business study, etc.);(4) building a high qualified teaching staff, improving the teaching system;(5) building a post competency appraisal system, improving the teaching quality.(6) communication and discussion between teachers and students. Through the above measures, the combination training mode of pediatric internship teaching and resident standardization training, can constantly explore new thoughts, improve the quality and standard of standardization training for clinical practice students, make the medical school education move on a step, improve the overall level of medical education.
