自拟固本平喘化痰方对慢性阻塞性肺疾病稳定期疗效及实验室指标的影响研究

Study on effect of self-made Guben Pingchuan Huatan decoction on curative effect and laboratory indexes of patients with chronic obstructive pulmonary disease

目的探讨自拟固本平喘化痰方对慢性阻塞性肺疾病稳定期疗效及实验室指标的影响。方法选取该院于2014年9月至2016年9月收治的慢性阻塞性肺疾病稳定期患者98例,按照随机数字表法分为观察组49例与对照组49例。对照组给予噻托溴铵治疗,观察组在对照组基础上联合自拟固本平喘化痰方治疗。两组疗程均为3个月。比较两组治疗疗效,治疗前后肺功能、6min步行距离(6MWT)及血清炎症因子水平变化。结果观察组治疗总有效率高于对照组,比较差异有统计学意义(P<0.05);两组患者治疗后一秒用力呼气容积(FEV1)预计值、最大呼气流量(PEF)、FEV1/用力肺活量(FVC)增加,观察组患者治疗后FEV1预计值、PEF、FEV1/FVC高于对照组,比较差异有统计学意义(P<0.05);两组患者治疗后6MWT增加,观察组患者治疗后6MWT高于对照组,比较差异有统计学意义(P<0.05);两组患者治疗后IL-6、IL-8、MMP-9水平降低,观察组患者治疗后IL-6、IL-8、MMP-9水平低于对照组,比较差异有统计学意义(P<0.05)。结论自拟固本平喘化痰方对慢性阻塞性肺疾病稳定期疗效明显,且可明显改善患者肺功能,降低血清炎症因子IL-6、IL-8、MMP-9水平,从而减轻患者炎症状态。

Objective To investigate the effect of self-made Guben Pingchuan Huatan decoction on curative effect and laboratoryindexes of patients with chronic obstructive pulmonary disease. Methods 98 patients with chronic obstructive pulmonary disease（COPD） in stable stage in our hospital from September 2014 to September 2016 were selected and divided into observation groupwith 49 cases and control group with 49 cases according to the random number table method. The control group was given tiotropi-um bromide treatment, while the observation group were given the basic treatment the same as the control group with the combination of self-made Guben Pingchuan Huatan decoction. Two groups were treated for a period of 3 months. The therapeutic effects ofthe two groups were compared, and the changes of pulmonary function, 6MWT and serum inflammatory factors before and aftertreatment were compared. Results The total effective rate in the observation group was higher than that in the control group （P〈0. 05） ; FEV 1 expected value,PEF and F EV 1/FVC value of the two groups after treatment were increased （P 〈0. 05） ; FEV 1 expected value,PEF and FEV 1/FVC value of the observation group were higher than those of the control group after treatment,and thedifference was statistically significant （P〈0. 05） ; 6MWT of two groups after treatment was increased （P 〈0. 05） ; 6MWT of theobservation group after treatment was higher than that of the control group, and the difference was statistically significant （P〈0. 05） ; IL-6 ,IL-8 and MMP-9 levels were decreased after treatment in two groups （P 〈0. 05） ; IL-6 ,IL-8 and MMP-9 levels of theobservation group were lower than those of the control group,and the difference was statistically significant （P 〈0. 05）. ConclusionThe self-made Guben Pingchuan Huatan decoction has a remarkable curative effect on chronic obstructive pulmonary disease,andcan significantly improve the pulmonary function of patients？and lower serum inflammatory cytokines IL-6 ,IL-8 and MMP-9 levels？thereby reducing the inflammatory state of patients.
