摘要
目的探讨癫痫患儿CD19、CD20、CD3-CD16+CD56+(NK细胞)和CD3+CD16+CD56+(NKT细胞)的变化情况和临床意义。方法选取2013年1月至2016年3月中山市人民医院儿童神经专科确诊的125例癫痫患儿作为癫痫组,另选取125例健康儿童作为对照组,流式细胞仪检测两组研究对象外周血CD19、CD20、NK细胞和NKT细胞水平。结果癫痫组CD19、CD20、NK细胞和NKT细胞水平均高于对照组,差异有统计学意义(P<0.05)。ROC曲线分析,CD19最佳截断值为9.4时,曲线下面积为0.903,灵敏度为91.7%。CD20最佳截断值为7.750时,曲线下面积为0.873,灵敏度为91.7%。NK细胞最佳截断值为8.55时,曲线下面积为0.805,灵敏度为95.8%。NKT细胞最佳截断值为0.350时,曲线下面积为0.769,灵敏度为91.7%。结论 CD19、CD20、NK细胞和NKT细胞可作为癫痫患儿免疫状态监测的重要指标。
Objective To investigate the changes and clinical significance of CD19 , CD20, CD3 CD16^+CD56^+ (NK cells) and CD3+CD16+CD56+ (NKT cells) in the children with epilepsy. Methods 125 cases of children with epilepsy children confirmed by Pediatric neurology specialists in Zhongshan Peopled Hospital from January 2013 to March 2016 were selected as epilepsy group, while 125 cases of healthy children were selected as control group,and the levels of CD19,CD20,NK cells and NKT cells in peripheral blood of two groups were detected by flow cytometry. Results CD19, CD20, NK cells and NKT cells levels in epilepsy group were higher than those in control group?and the difference was statistically significant (P〈0. 05). For ROC curve analysis CD19 optimal cutoff value of 9. 4, the area under the curve was 90. 3% , with 9 1 .7% sensitivity. CD20 optimal cutoff value of 7. 75 , the area under the curve was 87. 3 % , with 91 .7% sensitivity. NK cells optimal cutoff value of 8. 55 , the area under the curve was 80. 5% , with 95.8% sensitivity. NKT cells optimal cutoff value of 0. 35, the area under the curve was 76.9%, with 91.7% sensitivity. Conclusion CD19,CD20,NK cells and NKT cells can be used as important indexes for monitoring immune conditions in children with epilepsy.
