腹腔镜阑尾切除术治疗腹膜后阑尾炎

LAPAROSCOPIC APPENDECTOMY FOR RETROPERITONEAL APPENDICITIS

目的 :探讨腹膜后阑尾炎的腹腔镜阑尾切除术的可行性。方法 :回顾分析 2 8例腹膜后阑尾炎腹腔镜阑尾切除术 ,选择同期 2 5 2例非腹膜后阑尾炎腹腔镜阑尾切除术作对照 ,比较术中术后情况。结果 :腹膜后阑尾炎腹腔镜阑尾切除术组手术时间为 30± 8min较非腹膜后阑尾炎腹腔镜阑尾切除术组 2 2± 6min延长 (P <0 .0 0 1) ,术中出血、术后肠功能恢复 ,术后住院时间两组类似。结论 :腹膜后阑尾炎的腹腔镜阑尾切除术可行 ,无须中转开腹 ,但手术时间稍长。

Objective:To access the effect laparoscopic appendectomy for retroperitoneal appendicitis .Methods:Twenty-eight patients with retroperitoneal appendicitis and 252 patients with non-retroperitoneal appendicitis underwent laparoscopic appendectomy were reviewed retrospectively. The results were analyzed with t -test.Results:All cases were successfully done without conversion to laparotomy.The median operating time were 30±8 and 22±6 minutes respectively ( P <0.001) .The median blood loss, the recovery time of postoperative intestine function, the length of postoperative hospital stay were equivalent.Conclusions:Laparoscopic appendectomy for retroperitoneal appendicitis is enabled to be done safely without laparotomy.
