速度矢量成像技术评价系统性红斑狼疮患者颈动脉弹性 被引量:2

摘要 目的应用速度矢量成像技术(X-strain)评价系统性红斑狼疮患者颈动脉弹性。方法应用X-strain技术对比研究71例系统性红斑狼疮患者颈动脉弹性及形变能力的变化。71例患者根据病程分为SLE 1组(病史<5年,37例)及SLE 2组(病史≥5年,34例),38例健康体检患者为对照组。采用常规超声、X-strain技术评估患者双侧颈动脉,比较各组常规参数、X-strain技术参数之间差异和相关性。结果 SLE 1组与对照组内-中膜厚度(IMT)差异无统计学意义(P>0.05),SLE 2组分别与对照组和SLE 1组相比均具有统计学差异(P<0.05);三组间最大圆周应变及最大圆周应变率相比均有统计学意义(P<0.05),且随病程时间的延长逐渐减低;颈动脉IMT、X-strain参数之间具有明显相关性。结论速度矢量成像技术能够检测系统性红斑狼疮对颈动脉应变能力及弹性的影响,为评估和干预系统性红斑狼疮患者心血管事件提供可靠的指征。 Objective To evaluate the carotid artery elasticity changes in patients with system- ic lupus erythematosus (SLE) by using X-strain technique. Methods Seventy-one patients with SLE were divided into two groups based on different clinical process: SLE group 1 (duration 〈5 years, 37 cases), and SLE group 2 (duration ≥ 5 years, 34 cases). Thirty-eight healthy patients were in the control group. All patients with carotid uhrasonography were excluded plaques (thickness 0. 15 cm). Carotid arterial compliance and strain parameters were measured by X-strain technique, and the maximum value for Cs, Csr was obtained. Differences and correlation analysis in three groups of parameters were compared. Results There was no statistical significance of IMT between SLE1 and control group. The differences of IMT between SLE2 and SLE1 were statistically significant, and IMT of SLE2 were also statistically significant compared with control group. Comparison of Cs and Csr among three groups was significantly different. There was significant relationship among IMT, Cs and Csr. Conclusion With X-strain technology, the change of carotid arterial compliance could be early discovered. X-strain will become one of the important detection in the early intervention and preventing cardiovascular events for SLE patients.
