基于罗伊适应模式的护理干预应用于呼吸内科重症患者的效果

Effect of nursing intervention based on Roy adaptation mode for severe patients at department of respiratory medicine

目的 研究罗伊适应模式的护理干预对呼吸内科重症患者肺功能、生活质量及睡眠质量的影响。方法 选择 2015 年 6 月至 2016 年 12 月本院收治的呼吸内科重症且进行机械通气的 112 例患者，分为常规组和研究组，每组 56 例。常规组给予常规护理干预，研究组给予罗伊适应模式的护理干预，比较两组患者的肺功能、生活质量及睡眠质量。结果 护理前，两组肺功能、生活质量和睡眠质量差异均无统计学意义（均P ＞ 0.05）；护理后，研究组肺功能、生活质量和睡眠质量均得到改善，肺功能 FEV1、FVC、FEV1/FVC 分别为（2.39±0.54）、（2.75±0.63）、（73.44±5.19），均高于常规组 [（1.94±0.56）、（2.42±0.64）、（62.62±5.46）]，生活质量总体得分（72.9±8.3），显著高于常规组的（55.8±6.7），睡眠质量得分（1.25±0.25），显著低于常规组的（1.71±0.28），以上差异均有统计学意义（均P ＜ 0.05）。结论 对呼吸内科重症患者采取罗伊适应模式的护理干预，有效改善患者肺功能情况，大大提高患者生活质量和睡眠质量，利于患者康复。

Objective To study the effect of nursing intervention based on Roy adaptation mode on the lung function, quality of life, and sleep quality of severe patients at department of respiratory medicine. Methods From June 2015 to December 2016, 112 severe patients taking mechanical ventilation at department of respiratory medicine were divided into a routine group and a study group, 56 for each group. The routine group were intervened with routine nursing care and the study group with nursing care based on Roy adaption mode. The lung function, quality of life, and sleep quality were compared between these two groups. Results There were no statisitcal differences in the lung function, quality of life, and sleep quality between these two groups before nursing care （all P 〉 0.05）. After nursing care, the FEV1, FVC, and FEV1 / FVC were （2.39 ±0.54）, （2.75 ± 0.63）, and （73.44 ± 5.19） in the study group and were （1.94±0.56）, （2.42±0.64）, and （62.62±5.46） in the routine group, the total score of quality of life and the score of sleep quality were （72.9±8.3） and （1.25±0.25） in the study group and were （55.8±6.7） and （1.71±0.28） in the routine group, with statistical differences （all P 〈 0.05）. Conclusions Nursing intervention based on Roy adaptation mode for severe patients at department of respiratory medicine can improve the patients lung function, quality of life, and sleep quality and is good for their rehabilitation.
