实时三平面组织多普勒成像评价正常成人室壁运动同步性

摘要 目的:应用实时三平面组织多普勒(Triplane-TDI)技术评价正常成人室壁运动的同步性及规律性,定量分析室壁运动速度和时间参数,为临床提供准确评价室壁运动同步性的方法。方法:收集健康体检者120例,应用Triplane-TDI技术测量心室各节段的收缩峰值速度(Vs)、舒张早期峰值速度(Ve)、舒张晚期峰值速度(Va)、收缩达峰时间(Ts)、舒张早期达峰时间(Te),计算收缩达峰时间的最大差值(Ts-max)和标准差值(TsSD);测量各节段的收缩、舒张早期达峰延迟时间并计算均值。结果:(1)左右心室同一室壁不同节段运动速度具有梯度性:基底段>中间段>心尖段(P<0.05);左室不同室壁同一节段运动速度也具有梯度性:侧壁>后下壁>前间隔(P<0.05)。(2)Ts、Te在左室各壁以及同一室壁不同节段均无显著性差异(P>0.05);右室基底段与后室间隔基底段之间在收缩达峰延迟时间上存在显著性差异(P<0.05)。(3)左室12节段收缩达峰时间计算存在良好同步性:Ts-SD<33 ms,Ts-max<100 ms,Te-max<100 ms。结论:Triplane-TDI技术可以定量、准确、快捷的评价室壁运动同步性,具有广阔的临床应用前景。 Objective: Using real-time tri-plane tissue Doppler( Triplane-TDI) technical to evaluate the wall motion in normal adults,quantitative analysis of the wall motion velocity and time parameters and rovides an accurate method to assess the wall motion synchronization for clinical.Methods: Collected 120 cases of healthy persons Measuring the peak systolic velocity( Vs) of each segment,peak early diastolic velocity( Ve),systolic peak time( Ts) and the standard difference( Ts-SD) of each segment of the ventricles by Triplane-TDI; Measurement of the systolic peak early diastolic segment time delay and calculate the mean value.Results:( 1) Left and right ventricular wall motion with a velocity gradient of different segments: the basal segmentmiddle segmentapical segments( P〈0.05),the same segment of the different wall in left ventricular also exist velocity gradient: the lateral wallposterior inferior wallseptal anterior ventricular( P〉0.05).( 2) There was no significant difference on Ts and Te in left ventricular walls and different segments of the same wall( P〈0.05); There were significant differences between the right ventricular basal segments with basal septal segment on the delay time to peak contraction( P〈0.05).( 3) There is a good synchronization on systolic peak time of LV 12 segments as Ts-SD33 ms,Ts-max 100 ms,Te-max 100 ms. Conclusion: Triplane-TDI technology is a quantified,accurate and efficient method to evaluate the wall motion synchronization and has broad clinical application.
