自动插管补偿联合压力支持通气对慢性阻塞性肺疾病患者呼吸力学的影响 被引量:1

Effects of Pressure Support Ventilation and Automatic Tube Compensation on Respiratory Mechanic in Chronic Obstructive Pulmonary Disease Patients

摘要 目的观察压力支持通气模式(pressure support ventilation,PSV)联合自动插管补偿(automatic tube compensation,ATC)与单纯PSV模式对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者呼吸力学的影响。方法 21例COPD患者按随机顺序分别采用PSV+ATC模式和PSV模式进行治疗,比较两种模式对潮气量、无效呼吸次数、流速-容量(Flow-Volume,F-V)曲线等呼吸力学参数的影响。结果与PSV模式相比,PSV+ATC模式时COPD患者的潮气量显著增加,高潮气量报警次数和无效呼吸次数明显增多,F-V曲线的初始吸气速度显著增大,组间比较差异具有统计学意义(P<0.05)。结论对于COPD患者,不推荐PSV与ATC联合使用,建议两种模式分别单独使用。 Objective To investigate the effects of pressure support ventilation(PSV)combined with automatic tube compensation(ATC)on respiratory mechanics in patients with chronic obstructive pulmonary disease(COPD).Methods Twenty-one COPD patients were allocated into PSV+ATC group and PSV group in random order based on different treatment modes.The respiratory parameters including tidal volume,ineffective inspiratory times and flow-volume(F-V)curves,were compared.Results Compared with PSV+ATC modes,tidal volume,high tide alarm and ineffective inspiratory times,and initial inspiratory speed of F-V curves of COPD patients all increased in PSV+ATC mode(P0.05).Conclusion It is recommended to use a separate mode for COPD patients instead of the combination of PSV and ATC.
