Orem护理改进模式对急性肺损伤/急性呼吸窘迫综合征患者预后的影响 被引量:6

The Influence of Orem Nursing Modified Model on the Prognosis of the Patients with Acute Lung Injury/Acute Respiratory Distress Syndrome

摘要 目的:探讨Orem护理模式对急性肺损伤(ALI)/急性呼吸窘迫综合征(ARDS)患者治疗过程中及预后的效果。方法:将150例患者随机均分为观察组和对照组,每组75例。对照组运用常规护理方法,包括常规机械通气(CMV)和开放性液体管理策略(OFMS),观察组给予Orem护理模式护理,包括俯卧位机械通气(PPV)和限制性液体管理策略(LFMS)。观察2组护理前后患者治疗过程中及预后的效果。结果:对照组比较,观察组的生活质量和护理服务满意度均有明显提高,差异有统计学意义(P<0.05),且俯卧位比传统卧位能明显改善ALI/ARDS患者的氧合(P<0.05),观察组QLQC-30生活质量各项评分均要显著高于对照组(P<0.05)。结论:实施Orem自护理论护理有助于改善ALI/ARDS患者的预后。 Objective: To discuss the effects of Orem nursing model in the therapeutic course and on the prognosis of the patients with acute lung injury(ALI)/acute respiratory distress syndrome(ARDS). Methods: All150 patients were randomized into the observation group and the control group,75 cases each group. The patients in the control group used conventional nursing methods which include conventional mechanical ventilation(CMV) and open fluid management strategy(OFMS), and the patients in the observation group were cared by Orem nursing model, which contains prone position ventilation(PPV) and limited fluid management strategy(LFMS). The prognostic effects and the effects of the nursing in the therapeutic course of the patients were observed before and after the nursing. Results: Compared with the control group, quality of life and nursing service satisfactory degrees of the observation group improved notably, and the difference had statistical meaning(P<0.05), and compared with traditional supine position, prone position could notably improve oxygenation of ALI/ARDS patients(P <0.05),the observation group was higher than the control group significantly in different scales of QLQC-30(P <0.05).Conclusion: Implementing Orem self-care theory is helpful to improve the prognosis of ALI/ARDS patients.
