2013年-2016年头孢类抗菌药物临床用药分析

Clinical medication analysis of cephalosporin antibiotics from 2013 to 2016

目的探讨医院常用头孢类抗菌药物用药现状和趋势,旨在为头孢类抗菌药物合理使用提供指导意见。方法选取医院药学部药品库计算机管理系统提供的2013年6月-2016年5月常用头孢类抗菌药物使用数据,对常用头孢类抗菌药物销售数量、销售金额、构成比以及用药频度(DDDs)等情况进行统计分析。结果医院2013年6月-2016年5月各年度头孢类抗菌药物中常用药品销售金额中首位为头孢硫脒注射液,三个年度头孢硫脒注射液销售金额分别为4287394.4、6917626.9、4584950.4万元,其分别占35.1%、45.4%和31.6%;头孢硫脒注射液销售数量以及比例均处于首位,头孢硫脒、头孢唑林以及头孢哌酮舒巴坦排序均靠前;2013年6月-2016年5月各年度中头孢硫脒注射液B/A(销售金额排序序号B/DDDs排序序号A)均为1(1/1,1/1,1/1),2014年5月-2016年5月头孢替安、头孢哌酮舒巴坦B/A比值接近1(6/6,4/3)、(2/2,3/2)。结论医院头孢类抗菌药物使用趋于合理和规范化,然而个别品种依然存在过度使用的情况,所以还需进一步加强抗菌药物的使用管理,以便促进临床用药有效、经济和合理。

OBJECTIVE To explore the status quo and trend of cephalosporin antibiotics in our hospital,so as to provide guidance for the rational use of cephalosporin antibiotics.METHODS The statistical analysis was made on the number of cephalosporin antibiotics sold,the amount of sales,the composition ratio,and the frequency of drug use（DDDs）from Jun.2013 to May 2016 by the drug library computer management system of the department of pharmacy of our hospital.RESULTS The first amount of sales of cephalosporin antibiotics in our hospital of each year from Jun.2013 to May 2016 was cefathiamidine injection,and the annual sales amounts of cefathiamidine injection were 42873.944,69176.269 and 45849.504 million yuan,the proportions were 35.1%,45.4% and31.6% respectively.The number and amount of sales of cefathiamidine injection were both the first,and cefathiamidine,cefazolin and cefoperazone sulbactam were ranked in the top place.The B/A ratios（B：order of sales amount;A：order of DDDs）of cefathiamidine injection in our hospital of each year from Jun.2013 to May 2016 were 1（1/1,1/1,1/1）,and the B/A ratios of ceftazidime and cefoperazone sulbactam from May 2014 to May2016 were close to 1（6/6,4/3）and（2/2,3/2）.CONCLUSIONThe use of cephalosporin antibiotics in our hospital tends to be reasonable and standardized.However,there is still excessive use of some kinds of antibiotics,and the use of antimicrobial agents is still needed to be further strengthened in order to promote the effective,economical and rational use of antibiotics.
